About the role and where you will be working:
In this role youll design and deliver personalised care plans reflecting each clients aspirations.
Youll oversee budgets coordinate services and ensure compassionate high-quality care that exceeds expectations.
Working closely with clients and stakeholders youll create meaningful outcomes that truly enhance everyday lives.
Please note this role is Max Term of 6 months with potential for extension
Well also be asking you to:
Develop and manage personalised care plans that reflect each customers needs goals and preferences.
Build and maintain strong relationships with clients families and key health or community partners.
Oversee budgets and monitor spending to ensure cost-effective high-quality care delivery.
Regularly review and adjust services to support evolving client goals and changing circumstances.
