Job Summary

The Clinical Coordinator Pharmacist is primarily responsible for conceptualizing developing initiating maintaining and continuously assessing clinical pharmacy services. The team member ensures optimal clinical care in the acute and residential/partial psychiatric setting which includes aspects of general medicine through assessment of patient-specific factors and collaboration with other healthcare professionals. The Clinical Coordinator Pharmacist is delegated the task of Residency Program Director to oversee aspects of the PGY2 Psychiatric Pharmacy Residency program and provides exemplary customer service to patients and staff while completing tasks efficiently and accurately.


Qualifications

Essential Job Functions

Clinical Pharmacy Programs (50%)

  • Oversees effective clinical pharmacy programs.
  • Ensures safe and effective completion of daily clinical activities of clinical pharmacists.
  • Works with the Director to interview and recommend hiring of clinical pharmacy staff.
  • In conjunction with the Director participates in training and evaluating staff of clinical pharmacists.
  • Develops implements and coordinates orientation initial training and continuous training programs for new and existing clinical staff.
  • Coordinates the functions and activities of the department to promote prompt accurate and efficient clinical pharmacy services.
  • Organizes workflow to optimize clinical services and provide quality care.
  • Promotes professional development of self and staff through in-service education or outside programs.
  • Maintains updates and promotes use of policy and procedures for pharmacy department for review by Director.
  • Guides clinical pharmacists interventions with other health professionals.
  • Oversees development of material for a variety of committees tasked with clinical content (e.g. P&T Committee Antimicrobial Stewardship etc.) and oversees departmental participation in these committee meetings.
  • Oversees formulary management in collaboration with the operations manager and med safety pharmacist.
  • Oversees recommendations for therapeutic alternatives as a contingency during drug shortages.
  • Maintains a mature professional interdepartmental working relationship with professionals and non-professionals; encourages an atmosphere of teamwork.
  • Maintains departmental records prepares summaries and reports as required.
  • Performs quality assurance tasks.
  • Completes assignments within established guidelines.
  • Coordinates and supervises periodic clinical staff meetings to discuss issues problems communicate changes and/or additional policy and procedures in the department or hospital.
  • Assists in establishing objectives for the department and coordinates pharmacy policies and systems affecting the pharmacy and other hospital departments.
  • Monitors minutes of relevant committee meetings to ensure communication and follow through on pharmacy tasks.
  • Participates in drug utilization reviews.
  • Remains current on all federal and state laws and regulations as well as clinical standards pertaining to medications and orders; and enforces their implementation.
  • Monitors pharmacy activities to assure compliance of staff with current pharmacy policies.
  • Provides professional and clinical decision making backed by good rationale and judgement and supported by accurate and current knowledge of pharmacy practices.
  • Responds to drug information/pharmacy policy requests from medical/nursing staff in a timely and accurate fashion.
  • Communicates with different disciplines regarding current or new procedures/policies.
  • Serves as a resource to physicians nurses pharmacy staff and other patient care staff for clinical pharmacy services.
  • Inspires confidence by being well prepared requests and gives information clearly concisely and calmly.
  • Shows concern by actively listening considers what is said and done and offers assistance.
  • Demonstrates awareness of need to present oneself in a positive and professional manner.
  • Responsible for promoting the hospital and departmental image and relationships.
  • Performs other duties as requested.

Clinical Patient Care Functions (35%)

  • Utilizes patient specific information when interpreting reviewing and approving medication orders to assess safety and efficacy including but not limited to:
    • Age
    • Sex
    • Diagnoses
    • Allergies
    • Sensitivities
    • Current medications
    • Height and weight (when necessary)
    • Pregnancy and lactation information (when necessary)
    • Laboratory results (when necessary)
  • Ensures appropriateness of drug dose frequency and duration for each medication ordered prior to approving. Clarifies any ambiguities with prescriber.
  • Documents patient care clinical work and medication interventions within the medical record.
  • Serves as a hospital resource for drug information and utilizes appropriate reference materials when necessary.
  • Responds to drug information requests with accurate information in a timely manner.
  • Assists in reviewing and revising the drug formulary along with guidelines for drug use.
  • Participates in educating nurses patients pharmacy technicians pharmacy interns and other pharmacists as needed.
  • Participates as needed in hospital committees such as Pharmacy and Therapeutics Committee Continuous Quality Improvement Committee Infection Control Committee Fall Prevention Committee etc.
  • Collects and assesses medication usage data.
  • Participates in the Departments clinical initiatives and programs including therapeutic drug monitoring antimicrobial stewardship anticoagulation stewardship and medication reconciliation.
  • Adheres to McLean Hospital formulary whenever possible and provides recommendations for therapeutic interchange as necessary.
  • Collaborates with other healthcare disciplines to develop optimal treatment plans for patients.
  • Serves as a respectable pharmacy liaison to other departments.
  • Provides educational lectures or written reports as required.
  • Suggests improvements to the pharmacy services provided.
  • Maintains clinical and professional competency.
  • Other tasks as assigned by Director of Pharmacy.
  • May execute operational or dispensing functions based on departmental needs.
    • Accurately prepares labels and dispenses medications based on medication orders according to Pharmacy Department Policies and Procedures and applicable laws.
    • Supervises technicians in preparation of medications and verifies accuracy of all prepared orders including hospital prepared unit dosed medications.
    • Verifies accuracy of technician prepared packaging.
    • Ensures dispensed circulating and stocked medication is in-date and intact.
  • Alerts Pharmacy Department administration of any clinical issues as they arise.

Pharmacy Residency Program Coordination (15%)

  • Delegated task of Residency Program Director (RPD) to ensure rotation scheduling development planning and successful program completion of pharmacy residents who report directly to the Director of Pharmacy.
  • Ensure compliance with ASHP residency standards for PGY2 Psychiatric Pharmacy Residency Program.
  • Serve as a resident preceptor.
  • Support development and organization of preceptors.
  • Ensure timely and appropriate feedback is provided to residents.
  • Coordinate yearly PGY2 resident application reviews interviews and rank order lists.
  • Assist preceptors in the development and maintenance of learning experience syllabi.
  • Maintain policies and procedures for the PGY2 psychiatric pharmacy residency program and ensure continuous quality improvement of the residency program.
  • Coordinate MGB PGY1 pharmacy resident rotations at McLean Hospital.
  • Coordinate Residency Advisory Committee.
  • Maintain eligibility and qualifications of RPD as outlined by ASHP standards

Quality Improvement Responsibilities

  • Continuously assesses and improves the performance of care and services provided.
  • Uses systematic methods to improve services plan changes and evaluate outcomes.
  • Demonstrates effective problem-solving techniques and promotes the same in others.
  • Maintains awareness and actively supports the mission goals and objective of the hospital and department through CQI activities.
  • Other tasks as assigned by Director of Pharmacy or Chief Pharmacy Officer.

Departmental Responsibilities

  • Attends and actively participates in team conferences and staff meetings/in-services.
  • Complies with annual training requirements.
  • Participates actively in committee meetings as applicable.
  • Participates actively in orientation and on-the-job training of assigned personnel.
  • Arrives and leaves at schedule times; adheres to break and lunch time; willing to work overtime when necessary.
  • Presents for work as scheduled; is rarely absent and never without legitimate excuse. Notifies supervisor well in advance of any absences or vacation time.
  • Requires no overtime for normal shifts.
  • Maintains a clean and professional appearance in both dress and grooming.
  • Follows instructions/policy carefully and discusses deviations from procedure with supervisor before acting.
  • Demonstrates organizational time management and efficiency skills by prioritizing orders and activities.
  • Demonstrates initiative and accepts increased responsibility.
  • Maintains the pharmacy in a clean and neat manner as determined by the Pharmacy administration.
  • Adapts to change in a constructive manner and demonstrates flexibility for extra activities and unexpected duties.
  • Adheres to all Pharmacy Department policies and procedures McLean Hospital safety rules Joint Commission standards and any other applicable regulatory agency.
  • Completes special projects on time.
  • Identifies own learning needs by seeking assistance as necessary.
  • Willingly completes other duties as assigned or requested.
  • Maintains unrestricted pharmacist licensure in the state of Massachusetts.
  • Other tasks as assigned by Director of Pharmacy or Chief Pharmacy Officer.

Customer Service Responsibilities

  • Performs all tasks demonstrating service orientation to fellow employees and staff members to provide excellent customer service.
  • Demonstrates supports and models the mission vision values and rules of conduct of McLean Hospital.
  • Responds to concerns or requests of nurses patients doctors and hospital staff.
  • Demonstrates appropriate judgment and communication skills when interacting with physicians nursing patients and other hospital staff.
  • Communicates written and verbal information clearly accurately and professionally in a courteous manner and tone to all parties.
  • Provides follow up as appropriate and follows through to ensure messages or instructions are understood.
  • Responds to needs via telephone or pharmacy window promptly if needed.
  • Cooperates with others within and outside of department and coordinates work efforts.
  • Other tasks as assigned by Director of Pharmacy or Chief Pharmacy Officer.
  • Maintains patient/employee confidentiality.

Job Qualifications

Education:

BS and/or PharmD from an accredited College of Pharmacy required.

License:

Current unrestricted pharmacy licensure in the Commonwealth of Massachusetts. Board Certification in Psychiatry required.

Experience:

Previous clinical hospital pharmacy experience required. Residency experience required. PGY2 residency in psychiatric pharmacy and at least 3 years of experience in psychiatric pharmacy or at least 5 years of experience in psychiatric pharmacy required.
