Case Managers provide comprehensive patient care education and case management services. Their work requires a working knowledge of social work case management and local social servicessystems/resources. Their typical responsibilities may include handling case assignments drafting serviceplans and coordinating and providing care reviewing case progress and determining case closure;advocating for clients and helping them achieve wellness and autonomy; recording case informationcompleting necessary forms and producing statistical reports. They may work independently or as amember of a team. They work under limited supervision resolving most standard to moderatelycomplex issues independently and referring highly complex or unique issues to a high level. CaseManagers typically report to a manager or director or in some areas may report to an administrator.
Typical Duties
Responsible for the collaborative process with the client/patient/individual family and otherproviders (health care social services school etc.) of meeting the individuals health and dailyliving needs through advocacy assessment planning implementation coordination monitoringand evaluation of options and servicesIdentifies potential high risk/complex cases and develops a case management plan thatminimizes these risksIdentifies and assesses community based services educational financial mental health andhealth care systems as it relates to the individual case management/care management plan
assuring their appropriate utilizationWorks closely with senior staff/manager to address problems and issues as it relates to the
provision of servicesMonitors patients health status compliance with medical appointments and monitortreatment recommendations from the medical provider on an ongoing basisAddresses the emotional and mental health care needs of the client and family affected by theirdiagnosis and ongoing medical treatment
Preferred Education Experience
Masters degree in related field and 0-3 years of relevant professional experience or equivalent
combination of education training and experience
Preferred Expertise
Considerable knowledge and skill in application of professional medical principles proceduresand techniques as well as the ability to maintain confidentiality of all patient relatedinformation according to federal and state guidelines and regulations
Considerable knowledge of local federal and state laws and regulations regarding individualswith disabilities
Demonstrate ability and desire to advocate for the needs of the client when faced withchallenges
Demonstrate excellent oral and written communication skills in daily individual interactions andin presentations to small and large groups
Possess interpersonal and conflict resolution skills
Demonstrate ability to analyze and solve problems and to adapt to changes in work roles and expectations in order to accomplish client or department goal
Demonstrated ability to work in and foster an environment of respect professionalism andcivility with a population of faculty staff and students from various backgrounds andexperiences or a commitment to do so as a staff member at VCU
