Scope of work:
In conjunction with the Director of Care Management Education and Development and site care management leadership coordinates the site educational plan for the department of Care Management focusing on the clinical social worker role. Support audiences include all Care Management staff physicians and department managers/hospital administration. Uses specific age and culture-related physical intellectual psychological and development attributes in the educational plans for staff. Work collaboratively with members of an interdisciplinary team in order to assist patients and families with complex psychosocial needs.
- Develops/implements the educational plan for the site department of Care Management
Site orientation of new staff which is comprehensive and individualized with one-on-one training for three or
more weeks.
Rounding and telephonic support of staff education needs and problem solving.
Ongoing education based on analysis of outcomes from external payor audits.
Education and support for implementation and ongoing use of new electronic medical record system and
supplemental ancillary computer systems
Collaborate with other corporate team and other site educators to Provide education and support as needed. - System Liaison for the education of social/psychosocial needs.
Coordinates educational support related to patient initial needs assessment discharge planning and addressing all forms of identified abuse/neglect adoption substance use disorders assisting with end of life decisions providing appropriate referral resources bereavement support adjustment counseling facilitating guardianships and behavioral health concerns (including psychiatric transfers).
Coordinates and facilitates with other hospital department to ensure readiness for audits.
Analyzes audit recommendations.
Reports outcomes and development/implementation of staff and Physician education as
needed. - Conducts department-specific and system-wide assessment for educational needs related to Care Management Compliance Monitoring and Education
May serve as the liaison for compliance coordination including developing
revising monitoring the system Compliance Plan.
Monthly auditing of Compliance Risk areas and identification of staff education and
documentation needs to ensure compliance.
Analyze and evaluate the effectiveness of all educational activities. - In conjunction with the corporate team conducts educational workshops to service lines physician groups and other hospital and audiences as requested
Develops informational social work orientation materials and/or other media to be distributed to internal/external audiences
Education of changes and requirements to target Care Management departments and Physician groups
Qualifications
- Required Masters Degree in social work.
- 3 years of relevant experience Must have 3 to 5 years experience in Care Management Social Work including discharge planning. Required
- Related position(s); education experience. Preferred
- Clinical Social Worker (LCSW) - State of Michigan Upon Hire required
