Trumen Physicians Associates is seeking an experienced and compassionate Physician to join our growing post-acute care team in Tyler TX. The Post-Acute Care Physician will provide in-person medical care to patients in skilled nursing and rehabilitation facilities. Responsibilities include managing acute and chronic conditions facilitating care transitions collaborating with facility staff and supporting value-based care initiatives.
Perform in-person assessments and manage treatment plans in skilled nursing facilities
Collaborate with interdisciplinary teams to ensure continuity and quality of care
Update documentation and EMR in a timely and accurate manner
Participate in Trumens value-based and transitional care programs
Build and maintain strong relationships with facility staff and administration
Be available on-site during scheduled days
MD or DO with a current unrestricted Texas medical license
Board Certified or Board Eligible in Internal Medicine Family Medicine or Geriatric Medicine
Minimum of 1 year of experience in post-acute geriatric or SNF settings preferred
Strong clinical judgment communication and organizational skills
Reliable transportation within the Tyler area
