Job Highlights:
The Stroke Coordinator is accountable for leading the St. Marys stroke program and is responsible for the care of stroke patients throughout the continuum. The Stroke Coordinator works collaboratively with the Medical Director of the stroke program to improve patient care. The Coordinator is the lead facilitator of the multidisciplinary team responsible for stroke care including hospital departments physicians nurses and all health care professionals. The coordinator is responsible for ensuring and leading St. Marys continued certification as a Primary Stroke Center.
Key Duties include:
A. Stroke policy and procedure development and review
B. Responsible for compliance with all Federal and State regulatory statutes related to Neuroscience and recognized standards of care for Joint Commission Disease Specific Care.
C. Improves clinical outcomes by ensuring ongoing data collection and interpreting the clinical data to facilitate process improvement
D. Prepare and present formal quality reports to hospital wide committees and boards
E. Implementation of quality improvement methodology to improve care
F. Initiate quality improvement projects
G. Lead stroke quality improvement teams/meetings
H. Concurrent chart review
I. Participation in the inpatient rounds for stroke
J. Responds to code strokes when possible and facilitates care of the stroke patient
K. Contributes to exceptional clinical outcomes by serving as a clinical resource for staff on the care of stroke patients and serves as a consultant and evidence-based practice expert
L. Provides clinical expertise to staff and offers stroke education to physicians departments nursing health professionals and EMS providers
M. Responsible for the development and implementation of a staff stroke education plan for the hospital
N. Develops and/or selects stroke education materials utilized for patient education
O. Ensures that patient educational materials are reviewed for health literacy and cultural competence
P. Provide community stroke education through lectures interviews and presentations to audiences
Q. Maintains relationships and communication with local health care facilities and EMS to improve the coordination of care for stroke patients in the region
The Coordinator works under the direction of the Vice President of Clinical Operations. **Neurology experience preferred.

REQUIRED PROFESSIONAL LICENSE AND/OR CERTIFICATIONS
Resuscitation Certification Requirement: Basic Life Support Healthcare Provider (BLS HCP) required within 90 day of employment. Other resuscitation certifications may be required dependent on department.
