The Ambulatory Care RN provides professional nursing care to patients in an outpatient setting. This role includes patient triage assessment care coordination education and assistance with procedures. The RN collaborates with the interdisciplinary team to ensure high-quality patient-centered care and supports continuity across the care continuum.
Key Responsibilities:Perform comprehensive nursing assessments including vital signs and symptom reviews
Provide telephone or in-person triage prioritizing care based on acuity
Assist with procedures (e.g. wound care injections EKGs IV therapy)
Administer medications and vaccinations per provider orders
Educate patients and families on medications disease management follow-up care and preventive health
Coordinate care with providers specialists pharmacies and community resources
Document all patient interactions and clinical tasks in the Electronic Health Record (EHR)
Monitor and follow up on test results lab work and imaging
Participate in quality improvement initiatives and maintain compliance with clinic protocols and regulations
Current RN license in MD or compact License
Associates or Bachelors degree in Nursing (BSN preferred)
1 2 years of nursing experience (ambulatory or outpatient care preferred)
BLS certification (ACLS or PALS may be required depending on setting)
Proficiency with EHR systems (e.g. Epic Cerner)
Strong critical thinking multitasking and interpersonal skills
Experience with chronic disease management is a plus
