Job Description

The Family Transition Coordinator (FTC) is responsible for providing family driven youth guided treatment from point of admission to discharge and throughout after care. The FTC is responsible for case planning to a successful 12 month after care plan. The FTC will work in a close collaboration with the residential clinician referring partners and family.

RESPONSIBILITIES:

While in Residential Care:

  • Assist the Clinician in coordinating client home visits sibling visits and provide follow-up as appropriate this will include calling families scheduling and arranging travel communication with supervisors and other program staff and tracking statistical information as required.
  • Assist the Clinician in providing other case management services including scheduling clients activities and making appropriate contacts with referring workers program staff etc.
  • Assist the Clinician in Case Reviews by sending out invitations compiling test results attending and participating in Case Review Meeting discussions and writing summaries.
  • Assist the Clinician in processing case related information including profile sheets for new admissions preliminary work on service plans follow-up calls for discharged clients maintaining up-to-date client files etc.
  • Assist the Clinician with linkages with the clients Goal Worker and other program staff including the Nurses Wellness Specialists School Coordinator etc.; work to keep all service providers informed regarding changes in programming special needs etc.
  • May be required to specifically monitor the progress of certain clients as assigned. This may include work with clients on community living skills.
  • Regular participation in supervision meetings and team meetings for the sharing of ideas feedback and department planning.
  • Work in collaboration with referring partners family and community resources to engage the client with community opportunities that will lend to successful reintegration into that community.
  • Will ensure the childs/family transition discharge plan is reviewed and updated during the quarterly team meetings.
  • Collaborate on efforts to find fictive kin when no identified family is available
  • Demonstrate strong commitment to achieving permanent connections for every youth to ensure permanency and a broad family/community support network is in place for each child served
  • Facilitate ongoing family voice and choice related to transition and permanency planning.
  • Designated to monitor supervised visits with family or delegate as needed
  • Research and complete additional service referrals in county of service as needed
  • Routine travel to and from family homes as required for client treatment and care
  • Regular predictable and reliable attendance is an essential function of this position. The employee must maintain a consistent work schedule and be available during the assigned hours to perform the duties of this role.
  • Other duties as assigned

After Care responsibilities

  • Follow the client into the community placement providing face to face contacts as required per the DHHS contract for 30 days up to 6 months dependent on level of aftercare
  • Providing case management services for client and family to help collaborating systems and partners work together
  • Send out invitations compiling test results attending and participating in Case Review Meeting discussions and writing summaries.
  • Being a liaison for ongoing educational resources and services
  • Complete monthly reporting as assigned
  • Other duties as assigned

QUALIFICATIONS:

  • Bachelors degree is required in a Human Service Field by an Accredited college or university
  • LBSW or ability to be licensed is preferred
  • Minimum of one year of experience with High-Risk population preferred
  • Active Drivers License for the State of MI
  • Subscription to the Statement of Faith of Wedgwood Christian Services and agreement with the goal purpose and objectives of the agency.

HOURS OF WORK: 40 hours Monday- Friday with evening availability weekends on occasion
