Job Description

Ready to accelerate your career in human services management Are you a leader who wants to make a difference in the lives of others As a Program Supervisor in our day program you will lead a team of 8-10 Direct Support Professionals while also managing a caseload of 25-30 individuals. This is the perfect position for someone ready for their first supervisory role or an experienced manager who enjoys the variety of combining supervision with hands-on our day program you will work a standard daytime work week at our White Bear Lake location. Keep your nights and weekends free!

Phoenix Alternatives Inc (PAI) is seeking applicants who are dedicated caring and passionate about supporting people with disabilities who want to make a difference.

As a Program Supervisor you will:

  • Supervise teach and coach Direct Support Professionals and Lead Direct Support Professionals according to DHS licensing requirements and PAI performance standards. Ensure professional and regular communication to ensure ideas concerns and recommendations are given due consideration.
  • Train and supervise program staff to 245D requirements and PAI policies and procedures.
  • Provide supervision support evaluation and development of participants needs as specified in 245D licensing standards PAI policies and other applicable rules and regulations.
  • Coordinate interviewing hiring on-boarding and retention of staff.
  • Oversee and author participant Coordinated Service and Support Plan Addendum (CSSPA) Individual Abuse Prevention Plan (IAPP) Self-Management Assessment (SMA) person-centered outcomes and provide direct observation of staff members to ensure effectiveness of service delivery.
  • Respond to identified support needs interests preferences and desired outcomes for each participant.
  • Oversee program staff to ensure adequate daily staffing.
  • Ensure participant personal care needs are met and that proper transfer/lifting procedures are followed.

Your experience skills and attributes should include:

  • Meet Designated Coordinator standards as established per 245D. Minimally have one of the following:
    • A Baccalaureate Degree in a field related to Human Services and one-year of full-time work experience providing direct care services to persons with disabilities or persons aged 65 and older;
    • An Associate Degree in a field related to Human Services and two-years of full-time work experience providing direct care services to persons with disabilities or persons aged 65 and older;
    • A diploma in a field related to Human Services from an accredited post-secondary institution and three-years of full-time work experience providing direct care services to persons with disabilities or persons aged 65 and older; or
    • Minimum of 50-hours of education and training related to Human Services and disabilities and four-years of full-time work experience providing direct care services to persons with disabilities or persons aged 65 and older under the supervision of a staff person who meets one of the qualifications in a-c above.
  • 21 years of age or older.
  • Pass an OIG and DHS background study and maintain approved status without continuous supervision.
  • Reliable transportation for work driving purposes a valid drivers license proof of valid car insurance ability to satisfy insurance criteria for driving history and permit driving record checks as needed.
  • Experience with and understanding of Minnesota Statutes Chapters 245A 245C and 245D.
  • Experience with program development training and quality assurance or any equivalent combination of experience and training that provides the required knowledge skills and abilities.
  • Leadership and/or management experience.
  • Experience in a Day Training and Habilitation program or other disability services.

Salary:

$47000-$56000 ($22.60-$26.92 hourly) depending on experience.
