Job Summary
We are looking for a compassionate PRN to join our growing team! The right candidate has a can-do attitude a friendly demeanor excellent interpersonal skillsand the ability to provide personal care support to our clients in their homes.
Responsibilities
- Visit patients in their homes for one-on-one visits travel time is included in the $60/hr
- Complete initial and ongoing assessments to identify the physical psychosocial and environmental needs of home health patients
- Assess patients and their environmentto develop a care plan in order for the caregiver to have a blueprint to know how to safely and competently take care of our patients
- Educate patients and their families on proper home health care strategies and procedures and make recommendations for devices or tools that might improve the patients quality of life
- Provide effective communication to patients/clients their family members team members and other health care professionals
- Fill out VA Nurse Assessment documents on supplied tablet same day as assessment. Upload Assessment to CRM.
- Keep timely communication with Executive Director on schedule for upcoming and current assessments.
- Notify Executive Director at the end of each pay period list of assessments completed.
Qualifications
- Graduate of an accredited Diploma Associate or Bachelors School of Nursing or vocational nursing
- Current state license as an RN
- Proof of current CPR certification
- Compassionate friendly demeanor
- Excellent communication skills
- Current drivers license and reliable transportation
- Ability to work a flexible schedule and travel locally
