Hospital Discharge Team - Bridging Care Ensuring Recovery
Were seeking an experienced and enthusiastic Assistant Team Manager to help lead our South Locality Discharge Service and play a key part in delivering efficient effective services for adults across Wiltshire.
Our team is part of a supportive county-wide discharge service. This role is based at Salisbury District Hospital and covers other settings including rehabilitation beds in care homes. We offer a strong network of support guidance and professional development to support you in this role.
As Assistant Team Manager you will support the flow between acute and community settings to achieve positive outcomes for customers. Central to this is embedding HomeFirst principles and ensuring timely safe assessments and support delivery. You will have a pivotal role in delivering a high-performing customer- responsive service. You will supervise and support Social Workers and Social Care Practitioners ensuring a culture of learning and development.
We are looking for a confident decision-maker who understands the importance of performance and brings energy enthusiasm and a passion for doing things differently to achieve the best outcomes. You will need strong refined leadership skills to support develop and motivate a team of highly skilled professionals.
You should hold a relevant professional qualification in social care or health -such as a degree or Diploma in Social Work or Occupational Therapy. A Level 5 management qualification (or equivalent experience) is essential along with at least two years of management or supervisory experience.

This position comes with a 7.5% market supplement payment. All market supplement payments are subject to review on an annual basis.
