Place of performance: Lebanon Veterans Affairs Medical Center (Lebanon VAMC) located at 1700 South Lincoln Avenue Lebanon PA 17042 and four associated Lebanon VAMC Community Care Clinics: York VA Clinic 2251 Eastern Blvd. York PA; Mechanicsburg VA Clinic 5070 Ritter Rd. Mechanicsburg PA; Berks VA Clinic 2762 Century Blvd. Wyomissing PA; and Lancaster VA Clinic 212 Willow Valley Lades Dr. Willow Street PA.
Scope of Care: Contractors physician(s) as appropriate and within scope of practice/privileging shall be responsible for providing Urology care including but not limited to:
Clinic and Surgical Care: Contractors physician(s) shall provide clinical urology services including surgeries consultations cystoscopies and other related urology procedures. Contractors physician(s) shall be present on time for any scheduled clinics/surgeries as documented by physical presence in the clinic or operating room at the scheduled start time.
Specialty Exams: The contractor shall provide direct patient care including specialty exams such as compensation and pension agent orange etc. as it pertains to urological conditions only.
Medications: Contractors physician(s) shall follow all established medication policies and procedures. No sample medications shall be provided to patients.
Discharge education: Contractors physician(s) shall provide discharge education and follow up instructions that are coordinated with the next care setting for all urology clinical or surgical patients.
Administrative: Estimated 5% of time not involved in direct patient care.
Quality Improvement Meetings: The contractors physician(s) shall participate in continuous quality improvement activities and meetings with committee participation as required by the facility Chief of Service Chief of Staff or designee.
Staff Meetings: The contractors physician(s) shall attend staff meetings as required by the facility Chief of Service Chief of Staff or designee. Contractor to communicate with COR on this requirement and report any conflicts that may interfere with compliance with this requirement.

Technical Proficiency: Contractors physician(s) shall be technically proficient in the skills necessary to fulfill the governments requirements including the ability to speak understand read and write English fluently. Contractor shall provide documents upon request of the CO/COR to verify current and ongoing competency skills certification and/or licensure related to the provision of care treatment and/or services performed. Contractor shall provide verifiable evidence of all educational and training experiences including any gaps in educational history for all contractors physician(s) and contractors physician(s) shall be responsible for abiding by the Facilitys Medical Staff By-Laws rules and regulations (referenced herein) that govern medical staff behavior.
QUALIFICATIONS:
Staff/Facility
License: The Contractors physician(s) assigned by the Contractor to perform the services covered by this contract shall have a current license to practice medicine in any State Territory or Commonwealth of the United States or the District of Columbia when services are performed onsite on VA property.
All licenses held by the key personnel working on this contract shall be full and unrestricted licenses. Contractors physician(s) who have current full and unrestricted licenses in one or more states but who have or ever had a license restricted suspended revoked voluntarily revoked voluntarily surrendered pending action or denied upon application will not be considered for the purposes of this contract.
Board Certification: All contractors physician(s) shall be Board Certified Urologist by the American Board of Urology (ABU) or the American Osteopathic Board of Surgery Certification in Urology and be currently certified in Basic Life Support through the American Heart Association (BLS). All continuing education courses required for maintaining certification must be kept up to date at all times. Documentation verifying current certification shall be provided by the Contractor to the VA COR on an annual basis for each year of contract performance.
DaVinci Robotics Qualifications: At least two of the minimum three surgeons must be robotically trained as follows:
Female Urology Specialty Requirements: At least one of the minimum three surgeons must be a female provider that is fellowship trained in sub-specialty of female urology. This requirement reflects the increased number of female Veterans being seen in VA. Lebanon VAMC recently hired a female Gynecological provider who is filtering many female patients to the Urology specialty for these conditions. Some have suffered Military Sexual Trauma and it has been relayed by patients and their treating physicians that these Veterans benefit from and are much more comfortable seeing a female provider for their unique needs. Lebanon VAMC currently provides this option of care for our female Veterans and considers it critical to maintaining this specialty service to our patients.
The female provider must have capability to provide female urology services to include but not limited to:

VA HOURS OF OPERATION/SCHEDULING:
Clinic and Surgical Services: Clinical and Surgical services shall include Robotic DaVinci Surgery and female-specific treatment and procedures as detailed in the price schedule. Regular clinic hours are 8:00AM through 4:30PM Monday through Friday.
Operating Room hours are 7:30AM through 3:30PM Monday through Friday. Saturday clinic services may be required to maintain acceptable access to care for veterans. The estimated need for Saturday clinic services is 30 hours per year.
On-Call Services: 24-hour on-call coverage for weekdays and 24-hour on-call coverage for weekends and federal holidays is required as detailed in the price schedule.
