About this Position

The Behavioral Health Care Manager plays a vital role in supporting and coordinating the mental and physical health care of patients enrolled in the OSU-CHS Collaborative Care Program. This position works closely with primary care providers mental health professionals and a multidisciplinary team to manage an assigned patient caseload. Responsibilities include conducting patient intakes delivering brief therapeutic interventions promoting health literacy and connecting patients with community resources.

This role involves both telehealth and in-person services and may include providing therapeutic services in addition to care management duties. The Care Manager will present cases during regular consultation meetings and communicate treatment recommendations to patients and providers.

About the Collaborative Care Model

Collaborative Care is a team-based population-focused approach to mental health care that integrates care managers primary care providers and psychiatric consultants. The Care Manager is a key contributor to the implementation and success of this model ensuring coordinated patient-centered care.

Required Qualifications

  • Masters
    • Master of Social Work (MSW) or equivalent licensure.
    (degree must be conferred on or before agreed upon start date)
  • Certifications Registrations and/or Licenses:
    • Licensed Clinical Social Worker (LCSW) or equivalent licensure.
    • Valid Drivers License
  • Skills Proficiencies and/or Knowledge:
    • Proven ability to collaborate and communicate effectively within a multidisciplinary team.
    • Strong interpersonal skills and the ability to maintain professional relationships with patients and colleagues.
    • Experience in screening and assessing for mental health and substance use disorders.
    • Knowledge of differential diagnosis and treatment planning for common behavioral health conditions.
    • Proficiency in engaging patients in therapeutic relationships both in-person and via telehealth.
    • Familiarity with evidence-based psychosocial treatments and brief behavioral interventions.
    • Basic understanding of psychopharmacology relevant to common mental health disorders within the scope of practice.
    • Experience with electronic health records and outcome tracking systems.
