Job Description

Position: Hospice/Palliative Care Physician with Hospitalist Responsibilities
Location: New Milford Connecticut
Schedule: Full-Time Monday to Friday
Setting: Inpatient Palliative Care & Hospital Medicine

About the Role
We are seeking a Hospice/Palliative Care Physician to join a collaborative care team in New Milford CT. This full-time role offers a unique blend of palliative medicine and hospitalist responsibilities providing continuity of care for patients with complex chronic or life-limiting conditions.

The ideal candidate will have a strong foundation in internal medicine or family medicine and a passion for providing holistic patient-centered care. You will work closely with interdisciplinary teams to address the physical emotional and spiritual needs of patients and their families.

Key Responsibilities

  • Provide inpatient palliative care consultations for patients with serious or terminal illnesses

  • Manage pain and symptom control complex decision-making and goals-of-care discussions

  • Support continuity of care as a hospitalist covering general inpatient medical patients when needed

  • Collaborate with nursing social work case management chaplaincy and specialty providers

  • Participate in family meetings advance care planning and discharge coordination

  • Engage in shared call coverage within the department

  • Document clinical encounters and treatment plans in a timely accurate manner in the EMR

  • Participate in hospital-wide initiatives to improve end-of-life care and patient experience

Qualifications

  • MD or DO with Board Certification in Internal Medicine or Family Medicine

  • Fellowship training and/or Board Certification/Eligibility in Hospice & Palliative Medicine

  • Eligible for medical licensure in the state of Connecticut

  • Hospitalist experience or strong comfort in managing acutely ill inpatients

  • Excellent communication and team collaboration skills

  • Deep commitment to palliative principles and compassionate care
