About the Role
We are seeking a Hospice/Palliative Care Physician to join a collaborative care team in New Milford CT. This full-time role offers a unique blend of palliative medicine and hospitalist responsibilities providing continuity of care for patients with complex chronic or life-limiting conditions.
The ideal candidate will have a strong foundation in internal medicine or family medicine and a passion for providing holistic patient-centered care. You will work closely with interdisciplinary teams to address the physical emotional and spiritual needs of patients and their families.
Key Responsibilities
Provide inpatient palliative care consultations for patients with serious or terminal illnesses
Manage pain and symptom control complex decision-making and goals-of-care discussions
Support continuity of care as a hospitalist covering general inpatient medical patients when needed
Collaborate with nursing social work case management chaplaincy and specialty providers
Participate in family meetings advance care planning and discharge coordination
Engage in shared call coverage within the department
Document clinical encounters and treatment plans in a timely accurate manner in the EMR
Participate in hospital-wide initiatives to improve end-of-life care and patient experience
Qualifications
MD or DO with Board Certification in Internal Medicine or Family Medicine
Fellowship training and/or Board Certification/Eligibility in Hospice & Palliative Medicine
Eligible for medical licensure in the state of Connecticut
Hospitalist experience or strong comfort in managing acutely ill inpatients
Excellent communication and team collaboration skills
