DEFINITION


HEALTH CLIENT SERVICES WORKER - PUBLIC HEALTH
Department of Health & Human Services

California Childrens Services operates within the Nursing Division of Public Health and is administered as a partnership between the County of Humboldt and the California Department of Health Care Services (DHCS). The HCSW position works collaboratively with a small team of Public Health Nurses a Medical Office Assistant and the Program Services Coordinator to ensure that children and youth ages 0 21 with rare and complex chronic medical conditions have access to high-quality specialty care. The HCSW position also provides support to the CCS Medical Therapy Unit and the Pediatric Cardiology Outreach Clinic.


What Youll Do:
This position is responsible for a variety of duties including;
  • Determine initial and ongoing program eligibility for children needing specialty medical care.
  • Work with families to complete enrollment requirements.
  • Explain the CCS program and policies to families.

Where Youll Do It:
This position is located at 908 7th St. Eureka CA 95501






A typical work schedule for this position is M-F; 8AM - 5PM

DEFINITION


Under general supervision performs interviewing to determine eligibility of clients for medical care rehabilitation and other services for such programs as California Childrens Services; performs related work as assigned.

DISTINGUISHING CHARACTERISTICS

This is an experienced class fully competent to interview clients and others and make determinations of eligibility for various types of medical and rehabilitative programs and/or referral to other types of service-delivery programs. This class is distinguished from the Client Services I/II classes in that the latter specifically provide intake services and eligibility determinations for County employment and training programs.

EXAMPLES OF DUTIES

  • Interviews and collects information and documentation from clients for use in determining eligibility for various medical or rehabilitative care or financial assistance for such programs as the California Childrens Services program.
  • Applies established rules and guidelines for eligibility determination; calculates family income insurance coverages etc. and verifies information with other agencies and sources; confers with supervisory or professional staff in complex cases.
  • Informs clients of eligibility status or suggests other options in instances of disqualification.
  • Coordinates and monitors medical service-delivery to ensure that appropriate services are received.
  • Prepares and organizes eligibility information for client file; prepares a variety of forms applications and records.
  • Enters a variety of information regarding clients into a computer system; updates information as necessary.
  • Confers with County staff and outside agencies to evaluate client needs and the type extent and cost of medical care.
  • Prepares and maintains a variety of reports and records related to client intake activities.
  • Coordinates efforts with other agencies to ensure maximum service and support for clients.

QUALIFICATIONS

Knowledge of:

  • Terminology and concepts related to health service delivery programs and medical treatment processes.
  • Basic interviewing techniques and methods.
  • Standard office practices and procedures including recordkeeping and the use of standard office equipment such as a word processor or personal computer.
  • Community resources available to clients.
  • Business English including spelling grammar and punctuation.
  • Basic business arithmetic.
Skill in:
  • Interpreting applying and explaining rules regulations and policies.
  • Conducting effective interviews to determine client eligibility for programs.
  • Preparing reports correspondence and other written materials related to intake activities.
  • Maintaining accurate records and files.
  • Operating standard office equipment and using a word processor or computer system.
  • Understanding and carrying out oral and written instructions.
  • Establishing and maintaining effective working relationships with those contacted in the course of the work.

Desirable Education and Experience:
A typical way to obtain the knowledge and skills outlined above is:
  • Equivalent to completion of two years of college or possession of an Associate of Arts degree with major coursework in psychology social services or a related field and two years of experience in interpreting applying and explaining complex laws rules or regulations to the public.

PROBATIONARY PERIOD

Persons appointed to regular County positions serve a probationary period. This is normally six (6) months but could be up to one (1) year. All designated safety employees serve a one-year probationary period.

DISASTER SERVICE WORKERS

All County of Humboldt employees are designated Disaster Service Workers through state and local law (California Government Code Section 3100-3109). Employment with the County requires the affirmation of a loyalty oath to this effect. Employees are required to complete all Disaster Service Worker-related training as assigned and to return to work as ordered in the event of an emergency.
