Description

The Clinical Dietitian Nutritionist 2 is responsible for providing Medical Nutrition therapy to patients in both the inpatient and outpatient setting to include nutrition assessment nutrition diagnosis and intervention in the management of patient care. Assesses nutritional needs and identifies nutrient requirements according to age disease and individual needs for all nutritionally at-risk patients. Develops implements and monitors nutrition care plans. Coordinates nutritional aspects of patient care with physicians providers and other health care personnel. Serves as a resource professional for Clinical Dietitian Nutritionist 1 and other health care professionals. Participates in medical rounds discharge planning and patient care conferences as appropriate for the clinical assignment. Acts as preceptor and assists in coordinating daily schedule/assignments for nutrition interns.

Responsibilities
  1. Patient Care
    1. Provides nutrition consultation and education for patients and their families who are screened at nutrition risk identified with nutrition risk and/or referred by physicians or other health care providers for nutrition-related problems chronic disease management of nutrition support.
    2. Acts a Specialist in Pediatric Nutrition for the Clinical Nutrition team.
    3. Applies the Nutrition Care Process including completion of the nutrition assessment determination of the nutrition diagnosis and developing and implementing evidence-based nutrition intervention(s).
    4. Performs nutrition focused physical exam (NFPE) during assessments on patients to determine risk and/or degree of malnutrition when indicated.
    5. Evaluates patient progress by monitoring adjusting and updating the care plan on a regular basis.
    6. Communicates the nutrition care plan with other members of the care team by effective verbal and written communication.
    7. Follows established department policies for nutrition screening and assessment.
    8. Provides inpatient and outpatient intervention and/or education by developing individualized nutrition care plans based on a thorough patient assessment and evaluation of patient nutritional needs.
    9. Applies knowledge of medical nutritional therapy and special age-specific nutritional needs to patient care plan.
    10. Addresses questions and concerns of patients and families community members and health professionals in accordance with evidence-based nutrition practice and critical thinking and assessment skills.
    11. Utilizes monitoring and evaluation techniques that consider the patients nutrition needs as well as cultural religious and ethnic concerns.
    12. Uses appropriate clinical documentation methods for all patient encounters.
    13. Documents the nutrition diagnostic statement in PES format (problem etiology and sign/symptoms) and provides supporting evidence within the progress note.
    14. Manages formula selections to best meet age and disease specific needs of patients in addition to financial considerations.
    15. Demonstrates accountability for the proper use of patients protection health information (PHI) and adherence to institutional safety practices.
    16. Assists with discharge planning along with the multidisciplinary team to ensure patients and their families have access to appropriate resources including WIC DME supplies etc.
  2. Education/Professional Development
    1. Serves as an educator and resource to medical staff and allied health personnel on nutrition issues including nutrition support therapy principles content and issues as needed for specific patient populations.
    2. Functions as clinical preceptor for the dietetic internship program and in a teaching role for other health care professionals patients and the community.
    3. Assists with daily assignments weekly schedules case study and research project preparation and the completion of competencies and other rotations-related paperwork.
    4. Interprets and presents current clinical knowledge standard of care and research to improve patient care and nutritional intervention.
    5. Participates in and completes activities/work projects individually or with assigned groups.
    6. Develops and implements as individualize portfolio plan for professional growth and development including participation in professional organizations and activities workshops seminars and staff development programs.
    7. Applies Scope of Practice Standard of Excellence Standards of Practice (SOP) and Standards of Professional Performances (SOPP) tools for credentialed nutrition and dietetics practitioners to determine competence advance practice and measure and evaluate an organizations programs services and initiatives.
  3. Performance Improvement
    1. Provides suggestions for program development and offer evaluations of existing programs to increase program performance and identify ways to improve the quality and efficiency of health care service delivery.
    2. Observes and adheres to all departmental and hospital policies and procedures and follows all safety quality improvement and infection control standards.
    3. Attends meetings as required and assists with improving the quality of meetings by taking an active role in meeting topics.
    4. Strives to promote the quality and efficiency of professional performance through participation in staff education programs approved continuing education courses and specialized skill training programs.
    5. Utilizes an organizationally approved nutrition care manual and evidence analysis library source to ensure evidence-based practices; keeps abreast of new developments and implements new clinical practices as appropriate.
    6. Assists in achieving compliance with regulatory agency standards including state and federal standards.
    7. Monitors clinical documentation methods and provides input in the development of documentation tools.
    8. Participates in department and hospital quality improvement activities.
  4. Other Duties as Assigned
    1. Performs other duties as assigned or requested.


Qualifications

Experience -Three years practicing as a clinical Registered Dietitian Nutritionist. Pediatric Nutrition (preferred)

Education - Bachelors Degree and completion of Accredited Dietetics Internship Program. Masters degree in nutrition-related field and/or Current Board Certification through Commission on Dietetic Registration or American Society for Parenteral and Enteral Nutrition.

Licensure - Louisiana Licensed Dietitian Nutritionist Registered Dietitian Nutritionist with Commission on Dietetic Registration
