- Provide personalized medical nutrition therapy assessing residents with acute and chronic conditions
- Educate and empower residents with tailored nutrition plans to enhance their well-being
- Collaborate with a skilled team including nursing dietary staff speech-language pathologists and medical professionals
- Monitor and assess residents with weight changes enteral feeding dialysis and wounds ensuring optimal care
- Develop and implement individualized nutrition intervention plans aligned with the Nutrition Care Process
- Offer compassionate counseling to residents adapting teaching methods to meet their unique learning needs
Qualifications :
- Bachelors degree in Dietetics or related field required; Masters preferred
- State Dietitian licensure required
- Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential preferred
- Minimum of two (2) years of related experience
- Familiarity with PointClickCare and the MDS process is a plus
