Job Description:
Were hiring a compassionate and skilled Licensed Home Care Physical Therapist to deliver high-quality personalized therapy to patients in their homes. This position is ideal for a PT looking for flexibility autonomy and the opportunity to build lasting patient relationships.
Responsibilities:
- Provide in-home physical therapy evaluations and treatments
- Develop and implement personalized treatment plans
- Educate patients and caregivers on mobility safety and exercises
- Document patient progress and update care plans as needed
- Collaborate with interdisciplinary team members
- Ensure compliance with HIPAA state and federal guidelines
Requirements:
- Valid Physical Therapy license in the state of Maryland
- Degree in Physical Therapy (DPT MPT or BSPT)
- At least 1 year of physical therapy experience (home health preferred)
- CPR Certification
- Valid drivers license and reliable transportation
