Job Description
- Routine Follow-Up Visit: $25-30 per hour

Responsibilities:
- Provide skilled nursing care to patients in their homes in accordance with established care plans and physicians orders
- Monitor and assess patients health status and report any changes or concerns to the supervising Registered Nurse
- Administer medications and treatments as prescribed by the physician
- Provide wound care catheter care G-tube care and other nursing procedures as needed
- Educate patients and their families on their conditions medications and self-care techniques
- Collaborate with other healthcare professionals to coordinate and manage patient care
- Maintain accurate and up-to-date patient records including progress notes medication administration records and nursing assessments
- Adhere to all regulatory and ethical standards including HIPAA guidelines and infection control protocols
- Communicate effectively with patients families and other team members to ensure the best possible outcomes for patients
Requirements: *New Graduates Welcomed*
- Active and unrestricted LVN license in the state of California
- Minimum of 1 year of experience as an LVN preferably in a home health setting
- Have experience working with patients diagnosed withautism cerebral palsy down syndrome and/orepilepsy
- Have experience working in pediatrics
- Strong knowledge of nursing principles practices and procedures
- Excellent clinical assessment and critical thinking skills
- Ability to work independently and manage time effectively
- Strong communication interpersonal and organizational skills
- Must have a valid drivers license and reliable transportation
- Must have current BLS certificate
- Physical and TB clearance valid within the year
- Ability to complete documentation via agency EMR system
SHIFTS:Minimum of 6 hour shifts no more than 8 hours no holiday shifts
Morning Afternoon and Evening
To achieve a full 8-hour workday you may be assigned multiple patients and shifts typically vary in length
