Job Summary:
This Registered Nurse (RN) position requires providing skilled nursing care case management and hospice care to patients facing age-related frailty and serious illness. The RN will perform patient assessments administer medications develop treatment plans and collaborate with an interdisciplinary team. Strong communication and collaboration skills are essential. A minimum of two years of experience is required.

Responsibilities:
- Providing skilled nursing care to patients with advancing age frailty and serious illness.
- Performing patient assessments and developing treatment plans.
- Administering medications and treatments.
- Collaborating with the interdisciplinary team (physicians other nurses therapists etc.).
- Case management and hospice care.
- Maintaining accurate patient records and documentation.
- Communicating effectively with patients families and healthcare providers.
Required Skills & Certifications:
- Clinical assessment medication administration and treatment planning skills.
- Two years of minimum experience in hospital home health or hospice care.
- Interdisciplinary team collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Active Registered Nurse (RN) license in the state of employment.
- Bachelors degree in Nursing or related field.
- Drivers license car insurance and personal automobile for field work.
Preferred Skills & Certifications:
- Experience in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room settings.
Special Considerations:
- The role may involve working in a fast-paced environment and may require travel for home health or hospice care.
