The Intake coordinator is responsible for managing new clients into the integrated clinic. Serving as the first point of contact the intake coordinator facilitates timely access to services by coordinating assessment verifying eligibility with front desk staff and supporting clients engagement. This role ensures a smooth transition into care by working closely with clinical administrative and multidisciplinary teams across mental health and substance use. The intake coordinator will work with Hope Center and integrated division to streamline workflow from referral to intake.

- Conduct initial psychosocial assessment and clinical screenings per OMH and OASAS guidelines
- Determine preliminary diagnosis and level of care based on clinical findings and standardized screening tools
- Serve as the initial point of contact for new clients and collaborate across disciplines to ensure timely linkage to services.
- Assist with data collection and reporting
- Completes all documentation in the electronic health record (EHR within 48hours of contact adhering to all regulatory and billing requirements (OMH OASAS)
- Responds to all referrals within 24hours while working closely with ICL HOPE Center
- Works with front desk staff to ensure all intakes are scheduled weekly
- Support the clinics integrated care mission by identifying gaps in care barriers to engagement while improving opportunities.
- Collaborate with internal/external referral sources including hospitals shelters residential
- Maintain accurate and timely record of assessments screening and coordination activities
- Participates in quality assurance meetings and contributes to development of workflows and protocols that improve access and outcomes.
- Performs other related duties as required.

QUALIFICATIONS AND EXPERIENCE:
Must be LMHC LMSW in New York State. Two years of experience; Experienceworking with adolescents and adults with mental health and substance use disorder. Bilingual (e.g. Spanish/English) preferred but not required. Strong understanding of trauma-informed care child and adolescent development family systems and SUD.
