DescriptionThe Psychiatric Nurse Practitioner will be a member of the Interdisciplinary team whose purpose is to evaluate treat and plan for the aftercare of patients.
Responsibilities- Provide direct care to patient assigned to the team including appropriate and timely care and documentation.
- Complete admission assessment with a focus on comprehensive risk assessment for all new intakes and reassessment of patients in a timely manner consistent with policy.
- Conduct appropriate outreach and relevant documentation as per policy.
- Complete treatment plan documentation.
- Participate in all huddles to share information.
- Ensure appropriate use of de-escalation participate and provide leadership in behavioral codes.
- Participate in psychiatric codes medical codes in the clinic until code team arrives.
- Work with all teams to help patient move through appropriate level of care.
- Appropriate risk stratification and re-assessment of risk level.
- Participate in discharge planning and including referrals to alternative levels of care.
- Collaborate and coordinate with primary care and other medical specialists.
- Participate and conduct case reviews.
- Work closely with the collaborative physician.
Qualifications-Licensed as a Psychiatric/Mental health Nurse Practitioner
-Licensed Registered Nurse
-MS as a Psychiatric/ Mental Health Nurse Practitioner
-The candidate must have at least one (1) year of education and training and experience in psychiatry services
