Position Description: The Family Case Manager at Lighthouse Mission Ministries(LMM) Enhanced Shelter plays a vital role in providing compassionate and holistic support to families experiencing homelessness. The position is responsible for assessing the needs of families creating and implementing individualized service plans and connecting them with the necessary resources to help them transition to stable housing.
Compensation to Include: PTO (120 hours per year) & Paid-holidays (92 hours per year) Medical Dental Vision and Life Insurance for employee; 403(b) contribution and match up to 5% Employee Assistance Program Paid Training and Education Reimbursement.
Responsibilities:
- Conduct comprehensive assessments to determine the needs and goals of each family
- Provide for immediate basic needs upon intake
- Develop and implement individualized service plans that include short-term and long-term goals addressing housing employment education healthcare and other relevant areas as determined by a self-sufficiency matrix evaluation every 30 days.
- Monitor and evaluate the progress of families through a minimum of weekly 1:1 case management sessions adjusting plans as necessary to ensure successful outcomes.
- Maintain detailed and accurate case notes documentation and reports.
- Connect families with community resources including housing assistance employment services educational programs healthcare childcare and other support services.
- Collaborate with local agencies service providers and faith-based organizations to coordinate care and access resources.
- Provide emotional and spiritual support to families helping them navigate challenges and build resilience.
- Advocate on behalf of families with service providers landlords schools and other relevant entities.
- Coordinate events and outings for the Den participants
- Facilitate family meetings and support groups as needed.
- Represent LMM at community meetings and events to raise awareness about family homelessness and the organizations services.
- Respond to crises involving families providing immediate support and connecting them with necessary resources.
- Develop and implement safety plans for families experiencing domestic violence or other emergencies.
- Attend weekly case manager meeting
- Participate in reception functions as needed including phones security cameras and greeting ensuring the highest level of customer service at the shelter as needed.
- Monitor all safety and fire hazards as well as resident activity.
- Participate in prayer and Christian Devotionals in a group setting
- Maintain a positive pleasant and professional attitude. Work with other staff to model transparent and supportive Christian relationships which will serve as a leadership example for shelter guests.
- Oversee encourage and train volunteers and interns as needed.
- Work in alignment with the mission and values of LMM promoting a Christ-centered approach to service.
- Embrace and use LMMs grace-based accountability philosophy.
- Any duties as assigned.
Organizational Relationships:
- Works alongside a team of Crisis Services staff volunteers and interns.
- Represents LMM specifically Base Camp to other social service agencies.
- Reports directly to the Director of Crisis and Services Outreach.
Qualifications: The ideal candidate will have a combination of education life and work experience that demonstrates the ability to fulfill all aspects of the job description. To include at least 2 year of experience in Case Management rescue mission work or similar. Bilingual skills preferred.
- Experience in social services working with families experiencing homelessness and/or in the rescue mission environment preferred.
- A passion for serving Jesus and making the Gospel known through mercy ministry. The successful candidate will have an active church/prayer life and be in agreement with the Lighthouse Missions Statement of Faith and Personal Covenant
- Have knowledge of and preferably experience with issues of homelessness poverty addiction domestic violence and recovery.
- Able to use judgment to make independent decisions.
- Sensitivity to cultural diversity in all forms is required.
- Ability to successfully pass pre-employment criminal background screenings and comply with our drug-free workplace policies.
- High boundary setting and self-reflection skills.
- Basic motivational interviewing skills preferred.
- Ability to establish and maintain rapport with program residents and volunteers.
- Ability to remain calm and calm others in an emergency and/or confrontational situations.
- Ability to interact effectively as a team member and independently with LMM staff and with a diverse client base.
- Ability to use MS Word Excel Gmail and Google Calendar.
Physical Demands and Environmental Conditions:
- Ability to report as regularly scheduled to the work-site and remain awake and alert during shift
- Standing 40%; walking 30%; sitting 30%; ability to walk up and down stairs
- Medium to heavy lifting up to 35 lbs
- Possible exposure to a variety of illnesses
- Both inside and outside activities regardless of weather
- Ability to rapidly deal with emergency situations in an intermittently high-stress environment
