Job Description Summary

Cover consult pager during weekdays

Lead structured daily IDT rounds to ensure efficiency of IDT

Facilitate multidisciplinary goals of care meetings Promote identification of HCPOA and advance directive completion

Provide coverage for LVAD consults (required for LVAD program accreditation) and HCPOA requests when SW/Chaplain unavailable

Provide coverage for LVAD consults (required for LVAD program accreditation) and HCPOA requests when SW/Chaplain unavailable

System-Wide Impact:

Serve as a direct contact and coordinator of care for patients requiring general inpatient (GIP) level of hospice care

Coordinate with unit-based Case Managers to identify patients appropriate for consultation

Review patients who trigger as potentially benefiting from a palliative care consult and coordinate with primary team (e.g. COPD initiative) to improve access to and timeliness of PC consults

Facilitate opportunities for primary palliative care education to bedside staff and medical teams

Coordinate in-person and Telehealth inpatient PC to ambulatory PC transition (including patient/ caregiver education ensuring adequate medication supply)

Staff support for implementation of any guidelines unique to palliative care patient population

Additional Job Description

Graduated from an accredited school of nursing. Current licensure by the South Carolina State Board of Nursing. BSN preferred. Minimum of three years of medical practice/clinical management experience. Other experience may be substituted in lieu of practice/clinical management. Current American Heart Association (AHA) Basic Life Support (BLS) certification or American Red Cross BLS for Healthcare Providers certification is required
