Job Summary:
This Registered Nurse (RN) position requires providing skilled nursing care case management and hospice care to patients with advancing age frailty and serious illness. The RN will perform patient assessments administer medications develop treatment plans and collaborate with the interdisciplinary team. Strong communication and collaboration skills are essential. A minimum of two years of experience is required.

Responsibilities:
- Patient assessments and monitoring

- Collaboration with interdisciplinary team (physicians other nurses therapists etc.)
- Case management
- Hospice care (if applicable)
- Documentation of patient care
- Maintaining patient records

- Drivers license car insurance and automobile for field work (if applicable)
Preferred Skills & Certifications:
- Experience in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room settings.
Special Considerations:
- Weekend work is required.
- Floating between units is required.
- Block scheduling may be accommodated per manager discretion.
- Return to work (RTO) is required.
- The facility is an acute care hospital.
Scheduling:
- Schedule details will be determined after hire. Weekend work is required. The possibility of block scheduling will be considered on a case-by-case basis.
