Job Summary:
This role involves providing skilled nursing care case management and hospice care to patients facing age-related frailty and serious illness. The Registered Nurse will perform patient assessments administer medications develop treatment plans and collaborate with the interdisciplinary team. Strong communication and collaboration skills are essential.

Responsibilities:
- Patient assessments and monitoring.
- Medication administration and documentation.
- Treatment planning and implementation.
- Collaboration with interdisciplinary team (physicians therapists etc.).
- Case management and care coordination.
- Providing skilled nursing care in various settings (hospital home health hospice).
- Maintaining accurate patient records and documentation.
Required Skills & Certifications:
- Clinical assessment medication administration and treatment planning skills.
- Active Registered Nurse license in the state of employment.
- Bachelors degree in Nursing or related field.
- Minimum two years of experience in hospital home health or hospice care.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Drivers license car insurance and personal automobile for field work (if applicable).
Preferred Skills & Certifications:
- Experience in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room settings.
Special Considerations:
- On-call responsibilities are expected 1-2 days per week and rotating weekends.
- First-time RNs are accepted.
Scheduling:
- Schedule will include rotating weekends and on-call shifts (1-2 days/week). Specific shift assignments will be determined by the employer.
