LOCATION OF POSITION
The position will be based primarily on the Frederick Campus; however the coordinator will also carry a caseload of students on theColumbia Campus and must be able to travel between both locations as needed.

Main Purpose of Job
The Transition Coordinator assists Maryland School for the Deaf students and theirfamilies in obtaining services necessary to transition from middle school to high schooland to post-secondary setting whether for post-secondary education employment orlong-term support.
POSITION DUTIES
Develop coordinate and implement transition plans withinthe IEP.
Provide age appropriate transition assessment to studentsto support development of post-secondary goals in areas of employmenteducation training or independent living goals.
Meet with students quarterly to review progress towardsthe transition plan.
Provide opportunities to meet with college recruiters orrecruiters from vocational training programs and support students with theapplication process for college or training program of choice.
Develop provide and monitor transition activities inareas related to academic employment transportation activities of dailyliving and independent living skills.
Coordinate referrals complete applications and attendtransition related meetings with multiple adult agencies.
Disseminates information to staff students and parentsregarding student needs progress and resources.
The coordinator will be responsible for meeting withassigned students and completing required documentation for Medical Assistancereimbursement on a monthly basis.
Work closely with Work to Learn staff whoprovide Pre-Employment Transition Services to students.
MINIMUM QUALIFICATIONS
Education: Masters degree in counseling social work educationor a related field required.
Experience: Two years of satisfactory performance as a teacher or acounselor.
Knowledge of the Deaf culture and Deaf Educationrequired. Must be fluent in ASL and written English. Must be ableto demonstrate knowledge skills and abilities to provide Transition Services.
DESIRED OR PREFERRED QUALIFICATIONS
Experience with the Transition Process.
Experience working with Adult Agencies.
