We currently have an exciting opportunity for aComplex Needs Workerto work in the Norwich office as part of a multi-disciplinary team and in close partnership with Mental Health services including healthcare to assertively engage with people with physical and/or mental health issues and involved in substance.
The post holder will hold a small caseload and provide bespoke support to our Service Users whilst providing a range of flexible and effective interventions to engage with our Complex Needs Service Users.
Location: Norwich Norfolk
Hours: Full time 37.5 per week
Full Time Salary: 27861.26 - 32002.35 Dependent on experience (Based on full time hours pro rata for part time)
Responsibilities
About the role:
- Coordinate and leadasolution-basedapproach engaging Service Users inappropriate services
- Carry a caseload of complex clients developing personalised action plans
- Participate in multi-agency initiatives to reduce the level risk
- Work closely with the drug and alcohol teamsto ensure that substance misusers and problem drinkers can accesstreatment
- Work proactively to ensure that Service Users have access to ongoinghealthcare
About you:
- Experienceof workingin one or more of the following fields:substance misuse alcohol misuse mental health
- Experience of working with people who have multiple and complexneeds
- Knowledge and experience of multi-agency workingwith experience offacilitatingmulti-agencymeetings
- Excellent communicationnegotiationand advocacy skills
- Be able to build andmaintainrelationships with a wide rangeofservices
- Can work autonomously as well as part of ateam
- Good levels of IT literacy including the ability to use IT systems including MSOfficeapplications
- Able tocritically reflect and evaluate interventions service delivery and areasfordevelopment

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles the salary and payments will be pro rata based on contracted hours.
