To provide a full range of psychosocial assessment intervention and care planning to patients admitted to Surgical Oncology.
Responsibilities include:
Psychosocial assessments counselling transition planning advocacy and participating as a key member of the care team
Participate in the advancement of patient care through daily inter-professional rounds unit-based and organizational committees quality initiatives and practice-based research
Active member of the social work team and participate in team meetings and SW led initiatives
Qualifications/Skills
Masters of Social Work Required
Member in good standing of OCSWSSW
Minimum of two years experience in a health care setting required
Oncology experience preferred
Excellent assessment and intervention skills
Knowledge of ACP and GoC skills and framework in working with this population
Collaborative practice competencies
Knowledge of the health care system its legislation and community resources
Demonstrated problem solving and conflict management skills
Superior facilitation skills
Commitment to Student Education and IPE activities
Commitment to Person Centred Care
