JOB SUMMARY: The Senior Manager of Population Health Data reports to and works under the direction and supervision of the Executive Director of Data Analytics & Insights.
PURPOSE AND SCOPE OF THE POSITION
The Senior Manager of Population Health Data will lead a team that is responsible for the gathering of all population health data for the Six Nations community. The team will use epidemiological data collection methods to assist the Six Nations of the Grand River (SNGR) organization in understanding the population health status of our community. This role will also contribute to the work of ensuring that our services across all departments are properly addressing but also preventing the social determinants of health in the community. Expectations are to support the Executive Director of Data Analytics & Insights in the communitys population health assessment disease surveillance service impacts upon wellness and other health-related data collection functions.
In addition the Senior Manager of Population Health Data will support the Core Services departments in the implementation of the goals and objectives of the various departments while working within the Administration policies and procedures established for this type of work.

BASIC QUALIFICATIONS:
- Master of Public Health degree in Epidemiology Indigenous Health Biostatistics or other applicable graduate education with a strong knowledge of population health and 2 years of leadership experience
OR
- Bachelor of Public Health degree in Epidemiology Indigenous Health Biostatistics or other applicable undergraduate education with a strong knowledge of population health and 5 years of leadership experience in population health analytics and surveillance

REPORTING RELATIONSHIP:

KEY DUTIES & RESPONSIBILITIES:
Technical Functions:
- Plans and coordinates population health assessment and surveillance processes to provide timely information to the Executive Director of Data Analytics & Insights
- Develops a process to analyze the impact of the organizations services on the wellbeing of community members. This work will be done in close collaboration with the Senior Manager of Service Data.
- Analyzes and interprets data to assess changes in community health health behaviors preventive health practices and health care utilization relevant to population health and demographics
- Develops a surveillance system for local provincial national and international trends in epidemiological activity and reports in a timely manner similar to what would exist in a public health organization
- Reviews current community health data to enable and support decision-making processes for SNGR and its stakeholders; responds to requests from stakeholders when applicable
- Provides expertise to support the departments and their services in population health assessment and surveillance activities which is geared at generating scientific and technical knowledge for evidence-based communication decision-making planning and implementation of community health-based policies and programs.
- Oversees complex data retrievals to meet health information requirements from key stakeholders reporting obligations and to ongoingly assess the population health status of our community members
- Provides expert-level subject-matter expertise and resource support to SNGR colleagues
- Liaises as necessary with stakeholders and resources in various health sectors research and regulatory institutions and builds strong working relationships between these and SNGR
- Presents findings initiatives and relevant SNGR activities at local regional national and international forums as appropriate
- Liaises with government public health research and academic institutions to contribute to and keep appraised of best practices in surveillance methodology information and reporting tools
- Prepares and/or reviews scientific and technical publications of trends investigative findings and research initiatives to internal audiences and the broader public health community as appropriate.
- Compile and clean the population health data summaries we receive from external institutions
- Enter the community health data sets into centralized databases
- Support the development of primary population health data analysis
- Works closely with the Senior Manager of Community Data & Analytics in the development and monitoring of key performance indicators for the organization as it relates to our operational needs
- Provide advice to Directors and other staff on how to properly measure and report population health data
- Strong understanding of Indigenous data governance and how these values interact with external agencies
- Use their research and data analytical skills and knowledge to identify any community health data gaps within our current system
- Provide descriptive analyses of our population and service areas of interest
- Continuously innovate and find ways to improve our community health data system
- Research and suggest new data reporting procedures and protocols
- Proactively identify any barriers trends or potential risks of the community health data reporting process and recommend solutions to the Executive Director of Data Analytics & Insights.
- Analyzes the community health data framework and processes alongside the Executive Director of Data Analytics & Insights
- Assists the Executive Director of Data Analytics & Insights with specific population health data extraction projects as requested
- Assists the Executive Director of Data Analytics & Insights in annual evaluations of the population health reporting process to measure its efficiency
- Provide information to staff and leadership and act as a specialist in all matters relating to epidemiology and health data
- To keep aware of regional national and international issues which have a direct effect on epidemiological trends in the community
- To act as a resource person resolve programs and instruct others as appropriate
- Maintains community health data sets and other applicable files
Administrative Functions:
- Provide leadership to the Population Health Data Team as a subdepartment of the Data Analytics & Insights department.
- Completes annual performance evaluations of all staff who report to this role
- Reviews the approval of time requests from staff including the approval of pay period time cards within the UKG system.
- Develops reports and provides presentations as applicable or upon request from the Executive Director of Data Analytics & Insights or other leadership
- Maintains a standard of excellence in all work
- Work independently and be proactive on various projects seeking advice when needed to limit any potential risk
- Develop technical and process manuals as applicable
- Works with the Policy Department to develop applicable policies and procedures for ensuring population health data is protected and privacy measures are in place
- Maintains excellent time-management and organizational skills
- Prepare relevant reports and documents as may be required
- May be required to attend meetings with the Executive Director of Data Analytics & Insights as requested
Communications Functions
- Ensures lateral kindness in all interactions with coworkers and those external to the organization to ensure a safe workplace for all team members.
- Ensure open and clear communication with the Director of Wellbeing and the Six Nations of the Grand River Elected Councils Wellbeing Committee on matters related to the health of the community
- Supporting communication staff in providing risk communication for diseases and hazards for the awareness of staff and the public
- Communicate and prepare data reports and information for briefings presentations and responses to enquiries the media and the public
- Liaise with leadership frontline workers health care providers and other service workers in the community to work on priority issues related to public health/community wellness based on health status analyses of the community
- Liaise and interact with research partners and local agencies in the monitoring and coordination of epidemiological trends and biostatistics relevant to Six Nations
- Communicate population health data findings to appropriate stakeholders
- Create various forms of data visualization and reports for the organization including but not limited to infographics animations presentations etc.
- Assist Directors and those engaged in population health surveillance in the organization to ensure they are updated on new practices in accordance with new legislation and best practice
- Ensure that you reflect a high degree of professionalism in all communications interactions and system processes
- Liaise and interact with external agencies other community health data specialists data analysts and other stakeholders/advisers as requested
- Liaise with community agencies and the general public on behalf of the Executive Director of Data Analytics & Insights when directed
- Exercise discretion and sound judgement in responding to inquiries for assistance and/or information regarding our community health data reporting
- Maintains working knowledge with guidance from the Executive Director of Data Analytics & Insights of the Six Nations of the Grand Rivers services and operations
Other Functions
- Performs other job-related duties as may reasonably be required by the Executive Director of Data Analytics & Insights.
- To respond in a Community Emergency situation and be able to provide support for the Executive Director of Data Analytics & Insights the Chief Executive Officer or Alternate.
- Provides support for the Six Nations Emergency Response Plan by ensuring awareness of the plan and assistance as instructed by the Executive Director of Data Analytics & Insights as a member of the Emergency Control Group.
- It is the responsibility of the employee to be aware of and adhere to all policies and procedures including those that relate to client safety staff safety and risk management.
WORKING CONDITIONS:

Receives additional direction guidance encouragement as applicable; ensures work meets the over-arching needs of the organization.

Receives technical guidance and operational knowledge for the generation of holistic data relating to our community health status.

KNOWLEDGE AND SKILLS:

- Master of Public Health degree in Epidemiology Indigenous Health Biostatistics or other applicable graduate education with a strong knowledge of population health and 2 years of leadership experience

- Bachelor of Public Health degree in Epidemiology Indigenous Health Biostatistics or other applicable undergraduate education with a strong knowledge of population health and 5 years of leadership experience in population health analytics and surveillance
Other Related Skills:
- Strong understanding of Ontarios public health system as well as First Nations health systems in the province
- Strong understanding of population health concepts health determinants diseases injury risk factors and health care utilization
- Demonstrated understanding of issues of First Nations research data and information governance
- Demonstrated understanding of issues of personal and community privacy and ownership
- Ability to critically evaluate and synthesize relevant scientific literature
- Formal training and experience in epidemiology software programs
- Strong quantitative and qualitative research study skills
- Experience working both independently and in a team oriented collaborative environment
- Strong computer skills which include data entry
- In-depth understanding of data management (e.g. permissions recovery security and monitoring)
- Knowledge of best practices for data reporting
- An understanding of health equity including the social determinants of health and their impacts on health outcomes for Indigenous populations
- A strong population health lens in all work
- Knowledge of working with various statistical packages an asset but not mandatory
- Highly skilled with Microsoft Office applications especially Excel and Power Bi but also Outlook Word & PowerPoint

IMPACT OF ERROR:
Errors in judgment and in the conduct of duties will lead to loss of credibility poor public relations confusion duplication of effort and misinformation being given to the Executive Director of Data Analytics & Insights the Chief Executive Officer the Six Nations Elected Council and the public.
CONTROL:
Guiding principles set by the Executive Director of Data Analytics & Insights and the Administrative policies and procedures established by the Six Nations Elected Council for the entire organization as well as being aware of other legislative standards (e.g. Privacy Confidentiality etc.) provided by respective government agencies.
