Description

Position: Case Manager 2

Program: Coley House

Location: Hamden

Salary: $19.50

Schedule: Monday-Friday 7am-3pm

Program Summary:

A residential step-down program where pregnant and/or parenting women are required to participate in substance use community treatment while living in a safe and structured environment. Clients will meet with their Case Manager on a weekly basis and attend daily psychoeducational groups. The program is designed to support women as they return to their communities as a healthy and productive parent in recovery.

Position Summary:

The Case Manager 2 is responsible for providing staff support and direct client care for both the womens residential recovery short-term treatment facility and mens residential permanent supportive housing program in Bridgeport. Responsibilities include delivering comprehensive case management services to clients including intake assessments development and implementation of individual care plans and liaison with treatment providers and community resources to support client success. This position is non-exempt and works under the supervision of program management.

Working within our Family & Child Services the Case Manager 2 works with the Coley House staff to provide services to stabilize and reunite families experiencing complex issues such as substance use behavioral health matters and homelessness.

Requirements:

  • Associates Degree
  • 2 years relevant experience in the direct service and case management field
  • Valid Connecticut Drivers License
  • Regular and predictable attendance is required
  • The Connection Inc. complies with Executive Order 13G. Any employee hired on or after September 27 2021 must either be vaccinated against COVID-19 or apply for a medical or religious exemption and be approved.

Orientation:

If selected for employment with The Connection newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.
