Job Description

Salary: $55000

PRIMARY JOB FUNCTION(S):

  • Prepares profile information and paperwork for all new admissions and discharges.
  • Collects comments from Department Directors regarding the ability of the agency to meet the needs of the persons served.
  • Prepares profile information to be presented at Admissions committee meetings; may coordinate meetings when appropriate
  • Acts as the Agency liaison for DDA and representatives of other funding agencies to ensure a constant flow of referral candidates.
  • Interviews referrals for day program placement.
  • Conducts day program tours for persons served families and/or service coordinators.
  • Reviews initial referrals for Residential placement and forwards all required paperwork to the Residential Department.
  • Monitors vacancies screens initial inquiries mails application and medical forms and interviews applicants and families when appropriate.
  • Provides support to families and persons served during admissions and discharge process.
  • Provides advocacy services for persons served and families in order to gain access to the DDA system.
  • Completes data entry and uploading of documentation into appropriate systems for all new admissions. Forwards documentation to the records room.
  • Markets the Agency in the community by attending all service provider fairs.
  • Contacts all service coordinators on a quarterly basis with a description of all residential program vacancies.
  • Works with individuals and families to provide them information about the services Chimes offers and process to get these services.
  • Obtains necessary documentation for residential and day placements in a timely manner.
  • Ensures appropriate state and agency eligibility for all prospective admissions.
  • Obtains appropriate authorization prior to admission from other service providers.
  • Closes persons served file upon discharge.
  • Uses technology for the completion of specified job duties.
  • Attends work regularly according to assigned work schedule and in accordance with Agency policy.
  • Attends and participates in inservice training staff meetings and other activities to facilitate professional development.
  • Works cooperatively with others including all staff supervisors administrators co-workers individuals served community professionals customers vendors and the public.
  • Follows instructions and abides by Agency policies and procedures.
  • Is a positive role model for individuals served and Agency staff.
  • Assumes other duties responsibilities and special projects as needed.

SECONDARY FUNCTION(S):

  • Works cooperatively and effectively with Agency volunteers maximizing the volunteers experience while meeting the needs of individuals served and the programs.

REQUIREMENTS:

EDUCATION: Bachelors degree from an accredited college or university with a concentration in psychology social work human services or related field.

EXPERIENCE: Two year of admissions experience with an Agency serving people with intellectual disabilities. Less experience may be accepted with higher level of Education completed. Must have an acceptable driving record as determined by criteria established by the Agencys insurance carrier and by Agency policy.

NOTE: At the discretion of the Personnel Officer additional related experience and/or education may be substituted in lieu of the requirements specified above under Education and Experience.
