Lead Support Worker
Lymington Hampshire
Permanent full time (37.5 hpw) rota working
Pay 13.21 per hour (salary 25838 per annum) and great benefits including Health Cash Plan

Typical day as a Lead Support Worker (known by us as Coordinator)
- Creating person centred support plans and risk management plans with your customers
- Helping customers with daily living activities such as making meals shopping budgeting maintaining their tenancy and moving onto independent accommodation if they are able.
- Working with our Health Coach to support customers to achieve positive health outcomes or with our training officer to support our customers to access training education and employment.
- Carrying out risk assessments support planning goal setting and regular reviews.
- Cuppas chats and catch ups. Theres no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too.

You bring
- Passion to support our customers to live their best life working collaboratively with an eye for detail.
- Experience of creating person-centred support plans and supporting colleagues to work to the plans.
- Experience of coordinating and assessing customer referrals.
- The ability to workon your own initiative remaincalm under pressure and havea resilient approach
- To get from A to B youll need a vehicle insured for business purposes. The great news is that well pay your mileage!

Our team
Youll join our large Hampshire West Team - there are quite a few of us please dont worry we wont expect you to learn all our names on the first day. You will be based at Lymington in the New Forest but there will be some travel expected to team meetings and training; you may even want to get involved with our outreach work in the local area.
Our Management team include Emma Karen Ade and Lucy. Hampshire West are super passionate about what we do! Were really proud of the daily support we provide our customers as well as supporting each other and if there is cake or biscuits involved it is always a bonus!
Job details
- We work on a rolling rota pattern including 9am-5pm 10am-6pm 11am-7pm shifts. Our working patterns include some weekends and bank holidays.
- Where were flexible with you we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
- Although you may occasionally lone work we do get together regularly in the daytime for training and team meetings.
- Able to use technology for creating and updating support plans complete online learning and to collaborate with colleagues.
- Youll need an Enhanced DBS check done and we pay for that.
