Job Description
About the Role:
As a key member of our multidisciplinary team you will provide high-quality person-centred support across acute subacute residential aged care and community programs. Youll play a vital role in:
- Providing psychosocial assessments and interventions
- Supporting patients through transitions of care and discharge planning
- Advocating for client needs and linking with community services
- Working closely with patients families carers and healthcare teams
What Were Looking For:
- Demonstrated experience in a healthcare or community setting
- Strong communication advocacy and interpersonal skills
- A commitment to collaborative culturally sensitive and trauma-informed care
