Job Description

Job Title: Patient Experience Specialist
Location: Southfield Michigan (On-site)
Reports To: On-site Manager

Position Summary:

The ideal candidate for this role is an advanced critical thinker and problem solver who is passionate about improving patient care and outcomes. Under the supervision of the Manager you will play a vital role in facilitating seamless healthcare experiences by addressing patient barriers ensuring efficient care coordination and supporting processes related to acquisitions infusions and diffusions.

This position focuses on proactive patient engagement ensuring smooth transitions across different aspects of care and collaborating with providers and healthcare partners to enhance the overall patient experience.

Key Responsibilities:

  • Serve as a resource for patients ensuring all interactions are clear compassionate and solution driven.
  • Maintain and update accurate patient demographic information within the Electronic Health Records (EHR) systems.
  • Coordinate and facilitate the exchange of essential patient information between EPIC Health providers specialists pharmacies and community partners.
  • Assist patients in overcoming healthcare barriers by connecting them with necessary services and resources.
  • Provide comprehensive care coordination before and after office visits ensuring an efficient and seamless patient experience.
  • Support and oversee patient experience related to acquisitions infusions and diffusions. ensuring adherence to protocols and quality standards.
  • Drive patient and provider satisfaction by anticipating needs addressing concerns and delivering high-quality service.
  • Collaborate with cross-functional teams to optimize patient engagement quality of care and operational efficiency.
  • Contribute to process improvement initiatives and training efforts to enhance patient experience strategies.
  • Participate in continuous education and development programs focused on patient advocacy care coordination and healthcare operations.
  • Perform other duties as assigned including support at additional locations as needed.

Who You Are:

  • High school diploma or GED required; additional education or certifications in healthcare administration customer service or patient advocacy are a plus.
  • Minimum of 2 years of experience in a medical or healthcare setting preferred.
  • Passionate about patient advocacy service excellence and problem-solving.
  • Excellent communication and interpersonal skills with the ability to build trust with patients and healthcare providers.
  • Strong organizational and multitasking abilities capable of working efficiently in a fast-paced environment.
  • Proactive adaptable and innovative in identifying solutions to enhance patient care and satisfaction.
  • Tech-savvy with experience using Electronic Health Record (EHR) systems and other healthcare technologies.
  • A team player who thrives in a collaborative high-performance environment.

Additional Requirements:

  • Background checks are required for this role.
  • This is an on-site position located in Southfield Michigan. Candidates must reside in the Metro Detroit area.
