Job Summary:
The Department of Social Work at SUNY Downstate Health Sciences University is seeking a full-time Community Relations Assistant Mother Cabrini Health Foundation. The successful candidate will:
Report to the Social Determinants of Health (SDOH) Program Manager.
Advocate for community needs and resources.
Make referrals to community-based organizations for patient who screen positive for Social Determinants of Health (SDOH)
Follow up with patients as needed to ensure that needs are addressed appropriately.
Provide culturally appropriate health education and information
Assist individuals in understanding their health conditions and treatment plans
Facilitate communication between patients and healthcare providers.
Offer social support as needed. Coordinate and implement health promotion activities and programs.
Collect data and maintain accurate records of referrals
Refer individuals to appropriate healthcare and social services
Follow up with all referrals for community-based organizations for outcomes
Organize and participate in community events to raise health awareness
Support individuals. in navigating the healthcare system
Required Qualifications:
Excellent communication and interpersonal skills empathetic attitude with an ability to offer emotional support.
Passion for the proper care and well-being of patients.
Preferred Qualifications:
2 years of experience in a healthcare setting preferred.
Knowledge of healthcare systems insurance plans and patient resourcesand an understanding of medical terminology preferred.
Work Schedule:
Monday to Friday; 9:00am to 5:00pm(Full-Time)
