Description

Position Summary:

The Program Manager - Jail Services interacts with corrections staff and assists in the provision of care and process of release planning. Jail Services staff utilize the Sequential Intercept Model as language when discussing services and defining how it fits into the system of care.

Responsibilities:

Program Oversight

  • Provides clinical and day-to-day supervision of Jail Services staff including providing feedback that supports program operation while facilitating professional and personal growth of staff
  • Facilitates hiring of and conducts annual performance evaluations for Jail Services staff
  • Oversees case management and other direct clinical services for adults incarcerated in identified correctional centers
  • Works with the Diversion Team and other internal teams on a comprehensive plan for jail mental health services
  • Works with the Crisis Lead Manager to ensure that program operations meet contractual funder and WAC requirements for program staffing and service delivery
  • Attends supervisory and/or program staff meetings to ensure needs of the program staff clients and contract requirements are met
  • Oversees the quality assurance and continued quality improvement implementation process for the Jail Services program
  • Establishes and maintains working relationships with community agencies and meets with providers of other community resources when appropriate
  • Fulfills contractual obligations to program funders and identifies additional program sustainability opportunities
  • Completes all fiscal responsibilities as needed for the program

Jail Evaluation/Intervention

  • Works with the jails and corrections facilities to identify individuals with behavioral health conditions who may benefit from assessment and treatment services
  • Utilizes available resources to complete assessments and arrange for treatment services as available under the contract
  • Utilizes crisis intervention techniques for stabilization or ensures that these services are provided in a timely manner and escalates to a Designated Crisis Responder when needed.
  • Formulates clinical decisions and makes appropriate referrals
  • Utilizes necessary clinical assessment tools and screeners to initiate treatment planning with input of jail and other treatment team members devises an incarceration plan of care and release plan/follow-up
  • Completes assessments/reassessments as appropriate and documents accordingly

Symptom and Medication Response Monitoring (Intercept III-IV)

  • Conducts and documents ongoing assessments of client symptoms and their response to prescribed medications and shares that information with providers and jail medical staff
  • Facilitates access to support and education to encourage clients to take medications as prescribed
  • Utilizes Motivational Interviewing and psychoeducation to promote medication adherence and responds to clients concerns about medication side effects and facilitates access to and communication with medical staff as needed.
  • Promotes self-advocacy to encourage communication about concerns for medication adherence or side effects to encourage medication adherence

Case Coordination and Discharge Planning

  • Coordinates jail releases for adults provides input to other mental health agency staff regarding treatment and discharge planning for clients served
  • Brokers services for new clients awaiting placement on treatment team (i.e. appointment with physician intake into services and other voluntary treatment services etc.)
  • Coordinates release planning which may include residential and inpatient placements and referrals to medical providers or other community resources.
  • Establishes and maintains working relationships with community resources to initiate or coordinate patient treatment with other providers
  • Demonstrates a comprehensive knowledge of community services and resources (including peer natural and community supports) and actively provides a warm hand off for individuals to access them.
  • Builds on client strengths to design interventions and safety plans targeted to individual needs
  • Assist clients by helping arrange transportation acts as a liaison with other treatment agencies with applying for social services benefits food stamps and other forms of assistance such as SSI and Medicaid
  • Under the direction of the Crisis Lead Manager notifies the BH-ASO MCOs and provider agencies of clients incarceration and release/transition from jail plans.
  • Attend community stakeholder meetings to assist in care coordination and referrals to appropriate services

Documentation

  • Ensures that necessary documentation is completed to meet the conditions of the corrections contracts
  • Completes or ensures the completion of all required progress notes according to Agency and MCO requirements guidelines and procedures regarding format content and timeliness
  • Language used in progress notes is respectful clear descriptive and concise and client centered.
  • Progress notes accurately depict treatment modalities and interventions provided within the clinicians scope of practice and according to the treatment plan.
  • Progress notes accurately reflect and depict client status and progress per assessment and reassessment data problem statements and treatment plan goals and objectives.
  • Assures that services and documentation are in line with the policies procedures and requirements as contracted with managed care organizations

Qualifications

Position Requirements:

The following requirements are those that are normally required for performance of this position. Physical Requirements:

This position normally requires that physical demands of standing walking bending lifting or performing other work requiring low physical exertion talking and hearing on a regular basis to perform the job requirements. These physical demands are required up to 80% of the time. Position also requires ability to drive with adequate vision and skill.

Non-Physical Requirements:

Education

  • Masters degree in social work mental health counseling or a related field leading to eligibility for clinical licensure

Experience

  • 2 years of experience providing clinical treatment services preferred
  • 1 year of experience working with the justice system law enforcement or corrections preferred.

Special Skills

  • Strong clinical and safety assessment skills
  • Knowledge of community and state hospital system
  • Knowledge of community resources for stabilizing individual in the community following inpatient treatment
  • Possess and utilize effective verbal and written communication skills
  • Ability to be self-motivated work independently and as a team member
  • Ability to represent the agency in a professional manner within the community

Licensure Registration Certification

  • Must have or be on track for one of the following WA State licenses:
    • Licensed Agency Affiliated Counselor
    • Associate or full licensure as Mental Health Counselor or Independent Clinical Social Worker
  • Valid Washington State drivers license and minimum liability insurance required by WA State
  • Must be deemed insurable as determined by Catholic Charities liability insurance provider

Employment is conditional upon:

  • Being cleared by criminal background check and fingerprinting when require

Wage:$85000 - $101000 per year depending on experience
Sign On Bonus:$20000-$30000 depending on experience (paid in 4 installments for total commitment of 51 months)
Schedule:Monday-Friday 8am-5pm
