Job Description

The Greater Nashville Regional Council (GNRC) is seeking a social worker with supervisory experience to join its Department of Aging and Disability Services as Manager of the TennCare CHOICES Intake team.


The CHOICES Intake team helps to screen and assess adults who may be eligible for the TennCare sponsored CHOICES program funded through Medicaid. CHOICES provides nursing facility services and home and community-based services for adults 21 years of age and older with a physical disability and seniors (age 65 and older).


The CHOICES Program Manager oversees the teams activities to ensure that program goals or objectives are accomplished in accordance with prescribed priorities timelines and funding conditions.


Theresponsibilities of the teaminclude:

  • Inform older adults and caregivers about programs and services available to assist with long-term care transportation nutrition or other needs as appropriate;
  • Conduct telephone and in-person screenings and assessments with potential program recipients across the Middle Tennessee area;
  • Complete pre-admission evaluations and collect financial and medical history information from potential clients
  • Keep informed on aging and disability related issues and gather information about available resources and local service providers;
  • Conduct periodic customer satisfaction surveys to assist GNRC and TennCare with monitoring the quality of the process; and
  • Document methods actions and outcomes and other important program information within the appropriate systems.


The duties of the program manager include:

  • Supervise train and coach professional staff assigned to the CHOICES Intake team;
  • Oversee and help carry-out the responsibilities of the team including screenings and assessments customer satisfaction surveys and the submittal of required documentation to TennCare;
  • Ensure daily operations of the team are in compliance with organizational policies and program requirements
  • Assist in developing educational material and in conducting outreach to referring agencies clients and caregivers and other partners;
  • Monitor the performance of program staff and the team by tracking and reporting key performance indicators to the Director of Aging and Disability Services;
  • Gather and maintain program documentation data and other information to be made available for program monitoring and audits;
  • Track and resolve complaints and program deficiencies identified by GNRC management clients and caregivers partner organizations or TennCare;
  • Participate in industry associations and continuing education opportunities to remain current on emerging and best practices;
  • Serve as part of the GNRC management team with a focus on ensuring programs and initiatives are aligned and coordinated; and
  • Other duties and special projects as assigned by the Director of Aging and Disability Services.

QUALIFICATIONS:The ideal candidate will have a Masters degree in Social Work an active social work license or LPN working knowledge of Medicaid and the CHOICES program good verbal and written communications skills and at least two years of experience supervising the work of others.


REQUIRED SKILLS: Proficiency in Microsoft Office and Adobe Acrobat software is required.


WORK SETTING:This is not a remote position. The selected candidate will be expected to work onsite on core business days. Work from home flexibility is provided when possible at the discretion of the Director of Aging and Disability Services.


TRAVEL EXPECTATIONS: This position will require travel throughout GNRCs thirteen county region. A valid drivers license and access to personal transportation is required.


COMPENSATION:The minimum starting pay is $60000. Actual pay will depend on experience and qualifications. GNRC offers a fully paid pension through the Tennessee Consolidated Retirement System and provides an optional 401k with dollar for dollar match of up to 3% of gross salary.
