Job Description
Hospice of Marion County a member of Empath Health is currently seeking Care Navigation Supervisor to join our team.
The Care Navigation Supervisor:
- Supervises and leads a team of Care Navigators by providing oversight and coordination of their day-to-day work assessing performance and providing feedback to drive results.
- In addition this individual will work directly with community partners outside of the Empath Health service area to ensure smooth facilitation of patient transfers/travelers to our service area and optimal access to the Empath Health Integrated Network of Care.
Qualifications/Requirements:
- Education: Associates degree (A.A.) or equivalent from two-year college or technical school; bachelors degree preferred
- Valid Florida license as a RN or LPN
- Strong preference for minimum two (2) years of hospice experience as RN or LPN
- Valid Florida State Drivers license
- Continuing education: As required for licensure
- Knowledge of hospice regulations such as admission/qualification criteria reimbursement and levels of care preferred.
- Excellent positive relationship building outreach efforts problem solving organizational time management creativity interpersonal skills written and verbal communication a must.
- Strong computer skills including knowledge of Microsoft Office applications and Electronic Medical Record
