Description

US Heart and Vascular is seeking a full time Medical Assistant Care Manager to join ourVirtual Care Call Center in Irving TX.

Position Summary:

The Care Manager will perform telephonic Chronic Care Management Principal Care Management and Transitional Care Management services to coordinate care for patients with chronic cardiovascular diseases and manage their post-acute care.

Responsibilities & Duties:

Under the direct supervision of RN Care Managers practice physicians and advanced practice providers the MA is responsible for the following.

  • Outreaching referred and eligible patients to introduce care management services obtain verbal consent for enrollment and document in EHR per standard operating procedures
  • Make outgoing calls to patients to assist patients in managing their chronic diseases - including education about their conditions and treatment regimens medication management appointment management
  • Responds to incoming calls from enrolled patients instructing them on their plan of care and coordinating care with their cardiology care team
  • Monitor patient adherence to plan of care and progress toward goals in a timely fashion and facilitate changes as needed
  • Screens patient health and SDOH needs documenting and monitoring care according to implemented care plans to optimize health outcomes
  • Serves as an important liaison for the interdisciplinary team inclusive of facilitating coordination with other inpatient and outpatient care delivery organizations
  • Provides patient and caregiver education related to health conditions patient care transitions of care and ongoing chronic care management
  • Ensures all CMS documentation and billing guidelines are adhered to in EHR

Requirements:

  • Requires excellent clinical communication and organizational skills
  • Must be able to function independently within the limits of the position
  • Ability to function in a team environment
  • Able to demonstrate interpersonal skills to be approachable and understandable to patients families and care team members
  • Graduated from an accredited Medical Assistant program required
  • Medical Assistant Certification
  • Current BLS for Healthcare Providers certification
  • One year of relevant work experience in a physician practice or healthcare setting preferredbut not required
