Job Summary

Under general professional supervision performs lead behavioral health case coordination services including planning caseload management training organizing of BHS care coordination processes and treatment referral/disposition. Acts as a liaison to other care coordination services within the BHS continuum of care. Delivers services in accordance with the approved plan of treatment and plan of care. Assists behavioral healthcare providers in understanding significant social and emotional factors related to the patients behavioral health issues.

Essential Functions:

  • Acts as a liaison among care coordinators in BHS programs. Completes weekly check-ins and monthly team meetings. Sets BHS standards for care coordination and communicates with program supervisors and managers regarding care coordination initiatives. (25%)
  • Manages caseload of clients within the BHS continuum of care who meet criteria for care coordination services. (40%)
  • Evaluates emotional components impact of health issues and assists client to identify disruptive factors to establish treatment goals. Adheres to and complies with rules and regulations of the City Agency Department and Service. Practices in accordance with ethical principles established in NASW code of ethics and by Denver Health. (10%)
  • Assists clients to maximize adjustment to behavioral health diagnosis/illness/disability/trauma by addressing biopsychosocial issues and enhancing problem solving and coping skills. (10%)
  • Documented activity reflects patient involvement in goal setting with written information consent documented. Completes documentation of care coordination activities and interventions per established standards and guidelines. Prepares and submits data and information to behavioral health supervisor in a timely manner. (10%)
  • Develops knowledge base of risk factors for psychosocial functioning wellness and health promotion and provides 10basic education to patient. (5%)


Education:

  • Bachelors Degree Psychology Social Work Counseling or related discipline required.


Work Experience:

  • 2-4 years experience working as a Case Manager Counselor Therapist or closely related role required.
  • Prior experience working in a management role within the behavioral health field required.


Knowledge Skills and Abilities:

  • Proficient in crisis intervention techniques with an ability to spontaneously manage problems as they arise.
  • Ability to advocate for client needs; including resources and support.
  • Ability recognize analyze and solve a variety of problems.
  • Ability to be flexible and adaptable to meet clients needs.
