The department of Medicine division of Hematology has an exciting opportunity for a temporary full-time Patient Navigator to work onsite at the UHealth campus. The Patient Navigator facilitates patients ability to obtain services by identifying barriers to care coordinating the development of treatment plans and monitoring the implementation and follow-up of services ordered by caregivers.
CORE JOB FUNCTIONS
Department Specific Functions
The patient navigator/community health worker will be knowledgeable about community resources to assist families of children and young adults impacted by sickle cell disease. This person will be supervised by Dr. Ofelia Alvarez pediatric hematologist at the University of Miami and will work collaboratively with another community health worker a social worker and other members of the pediatric sickle cell program. It is expected that this hired staff will come to clinic once or twice a week to meet families in order to inform about available resources. Sometimes the patient navigator may visit patients in the hospital. The rest of the time this hired staff will be available by phone. Occasionally the patient navigator will have to visit patients homes such as in the case of patients who are difficult to reach.

Any appropriate combination of relevant education experience and/or certifications may be considered.
