Job Description
Under the direct supervision of the Quality Manager the Care Coordinator (CC) is primarily responsible for the coordination of care assisting patients through the healthcare system by acting as a patient advocate and navigator. It is imperative that a Care Coordinator maintain excellent customer service skills and treat all patients in a friendly helpful manner. This position must work as a team player provide help and support to providers and co-workers and demonstrate professionalism through adherence to Ogden Clinics mission vision and values.
