Permanent Supportive Housing Case Manager
Location: 241 Greenhouse Road Lexington VA
Employer: Rockbridge Area Community Services (RACS)
ROCKBRIDGE AREA COMMUNITY SERVICES RACS is currently seeking a motivated qualified individual to work in our coverage area of Rockbridge and Bath County and the Cities of Lexington and Buena Vista. RACS will provide training supervision and education opportunities specific to the position.

Position Overview:
This position requires an experienced Case Manager to work with individuals challenged by years of homelessness substance use and behavioral and physical health issues. The PSH Case Manager will manage cases of residents at scattered sites in the service area. The Case Manager will work with individuals to attain services and supportive housing; work with residents and other service providers to develop a plan of service to meet social health emotional and economic needs to combat homelessness; coordinate services using Housing First and Harm Reduction approaches. The ideal candidate is committed to creating a safe and comfortable environment tailored to meet individual needs and to promote housing stability. Employee required to travel and work in individuals homes and the community.
This employee reports to the Adult Community Based Services Manager for all case work and clinical duties. In carrying out assigned position responsibilities he/she receives limited supervision and is required to exercise sound judgment demonstrate initiative and maintain a high degree of confidentiality in accordance with RACS policies and procedures and HIPAA.
Qualifications:
- Bachelors degree in human services field required
- At least two years of work experience providing direct case management services is required
- Experience working with adults with serious mental illness chronically homeless adults and/or adults with substance use required
- Strong communication documentation and organizational skills
- Valid drivers license and safe driving record in accordance with RACS policies are required
Background Investigation Requirements: All applicants are subject to a background investigation. Investigations may include fingerprint checks (State Police); local agency checks; employment verification; verification of education (relevant to employment); and other checks based on the position.
Probationary Period: All persons that are newly hired rehired transferred promoted or demoted into a Rockbridge Area Community Services position must serve a new 6-month probationary period.
