Be a keyworker to a number of participants and complete all duties and responsibilities of the keyworker position
Be familiar with the individual goals and preferences of the people we support
Implement care programs for those in residential establishments
Connect the participants we support into the Operations program based on their individual interests
Recognise all support requirements for the participants we support
Be actively involved in the development implementation monitoring and review of PersonCentered Plans and NDIS plans using individualized planning approach
Assist the participants we support to work towards achieving their dreams and goals and support them to be included in their communities of choice
Plan and develop nutritious menus in conjunction with Allied Health recommendations and participants choice
Monitor all types of financial expenditure including house accounts and assist the Regional
Manager with expenditure and participants funding
Provide documented individualised support when preparing and administering medication in accordance with the Medication Administration procedure
Support individuals and the team to participate actively and consistently in the participant daily routine and activities such as cooking cleaning grocery shopping and personal shopping
Support each participant to continuously learn and ensure current skills are maintained wherever practicable
Provide companionship and support during daily activities
Where appropriate and necessary assist or supervise participants with supports such as personal care and physical assistance and ensuring each participants comfort and dignity is respected at all times
Support participant where possible in active support including; Cook and serve meals clean premises wash iron and perform other household tasks
Work with a team of health professionals family friends and carers to implementing a Program of Support
Assist in the development and implementation of participant Behaviour Support Plans
Support the team to appropriate strategies and document behavior tracking as required
Attend and coordinate participants Allied Health appointments update Medication Treatment Sheets and notifying Internal and External stakeholders of outcomes
Review and submit risk assessments to Regional Manager in accordance with participant obligations and NDIS Capability Framework requirements
Attend and contribute to participant Care Team Meetings
Comply with One Doorway Policies and Procedures as applicable to your role
Other duties as required by the business

What we are looking for
An entry level qualification of CERT IV in Disability or equivalent and a minimum of 2 year experience in the position of DSW or Allied Health worker
Able to cope with the physical demands of the job
Able to perform domestic duties efficiently
Commitment to the rights of people with disabilities to live dignified lives
Demonstrated ability to work and accept responsibility either individually or in a team
Patient flexible understanding supportive and caring nature
Strong computer written and verbal communication skills
Experience working within the disability or health care sectors
Valid drivers license
Desirable however not essential

Valid Drivers licence
