Job Description

Summary

The Psychiatric Social Worker I will join the Ellis Medicine PROS located within the State St Health Center. The mission of PROS is toprovide person-centered holistic recovery-oriented care so individuals can achieve their life employee will have the opportunity to provide direct care and advocacy for a diverse and vulnerable PROS team specializes in a wide range of treatment interventions including trauma-informed care and integrated mental health and substance use treatment.

EDUCATION AND EXPERIENCE REQUIREMENT(S):

  • Masters Degree in Social Work Mental Health Counseling or related field
  • Mental health experience strongly preferred

PRIMARY RESPONSIBILITIES OF THE POSITION:

  • Provides direct care to a diverse population of people diagnosed with Serious and Persistent Mental Illness (SPMI).
  • Carries an individual caseload of 25-35 participants and weekly group load of 10-20; the number will be calculated based on acuity and need of participants as well clinical/supervisory judgement.
  • PROS is primarily a group model program. Staff will be responsible for creating implementing and running groups on a 4-12 week cycle.
  • In addition Psychiatric Social Work I will provide evidence-based treatment such as CBT DBT ACT and other clinically appropriate interventions to participants on their case load and group load as clinically indicated.
  • Incorporate multiple pathways to recovery and addiction treatment by utilizing a stage-wise treatment model addressing physical emotional behavioral and spiritual barriers to healing and achieving life role goals.
  • Create a trauma-informed environment with a focus on safety and stabilization peer support and personal empowerment utilizing evidence-based curricula including the use of integrated body/mind techniques self-care and peer encouragement.
  • Advocates for an underserved population through coordination with primary care care management and other community partnerships and facilitates/attends community provider meetings as need.
  • Assess determinants of health and make referrals for appropriate services including but not limited to housing transportation medical providers and care management.
  • Attends individual supervision (frequency determined by supervisor) weekly group supervision and weekly team meetings with the all staff
  • Completes initial assessments on new participants within 45 days of admission and complete formal recommendations for continued treatment.
  • Utilizes the CASE (Chronological Assessment of Suicide Events) approach to assess suicide/homicide risk and safety to provide accurate interventions to mitigate harm to patient and community.
  • Submits all documentation within 72 business hours from the time of service; ensure weekly contact notes monthly collateral contacts progress notes quarterly treatment plans and annual relapse prevention are up to date with each billable service completing plan amendments and documentation updates as needed.
  • Adheres to all OMH Joint Commission Department of Health Medicaid & Medicare regulations.
  • Coordinates treatment with treatment team to ensure high quality care.
  • Provides services to participants who are on Assisted Outpatient Treatment or other court-mandated treatment through the County and/or judicial system; this includes writing the AOT orders for the treatment team submitting them to the county and coordinating/attending all AOT meetings
  • Provides group and individual caseload coverage when team members are out of the office including providing crisis intervention de-escalation and risk assessment to any participant who requires immediate assistance.
  • Provide guidance supervision and mentorship to program interns from institutions of higher education both locally and nationally
  • Perform other duties as assigned

Salary Range: $22.28-32.31 /hour Pay is based on experience skills and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
