Position Summary:

The Access and Support Program supports people under the age of 65 (55 for First Nations People) to access health mental health disability and other community-based services as identified through a case plan. The program works in conjunction with VincentCares Northern Community Hub services in assisting people who are homeless at risk of homelessness and in low-cost accommodation or those who are financially disadvantaged. The program provides short term that is tailored to meet the needs of the individual client to link people in with services necessary to maintain their independence in the community.


The Access and Support Coordinator (A&S) will undertake direct support and care coordination to assist clients in reducing barriers to mainstream and specialist services for people with complex health and other support needs. Service coordination aims to place consumers at the lead of service delivery ensuring that they have access to the services they need opportunities for early intervention health promotion and improved health outcomes.


Support is delivered through the development of a holistic goal orientated case plan creating linkages supporting person to access appropriate mainstream and specialist services. The Access and Support Coordinator undertakes their role from an outreach model and will support the range of needs and issues affecting people who are homeless or at risk of homelessness including mental health alcohol and other drugs disability justice and family violence. The Access and Support program receives referrals directly from clients access points and a broad range of community service.
The Access and Support Coordinator will under minimal direction and guidance regarding work priorities be required to undertake complex operational work as well as undertake planning and co-ordination of activities within the Outreach Support Services Team.

What We Are Looking For:

  • A minimum of a relevant tertiary qualification with 12 years of experience in the required field is preferred.

  • Candidates with substantial experience in the discipline but fewer formal qualifications will also be considered except where mandatory credential requirements apply.

  • Proven ability to engage professionally with clients maintaining clear boundaries and accurate case records with accountability.

  • Skilled in delivering holistic support for complex needs including managing case plans resources and budgets.

  • Strong commitment to social justice human rights and cultural sensitivity with the ability to manage conflict and challenging behaviours effectively.

  • Excellent organisational and time management skills to handle deadlines in a demanding high-volume environment.

  • Self-aware open to feedback and committed to continuous learning and reflection.

  • Proactive confident and capable of exercising sound judgment.

  • Strong communication skills including report writing and maintaining professional relationships.
