Position Overview:

The Community Nutrition Navigator works both independently and as part of a multidisciplinary team to help clients improve their health outcomes self-management skills and satisfaction with careparticularly related to nutrition and food security. This role supports patients in navigating complex medical and social service systems with an emphasis on addressing social determinants of health (SDOH) including access to culturally relevant food and nutrition resources. The Community Nutrition Navigator maintains up-to-date knowledge of internal NARA NW programs and community-based support facilitates referrals and ensures care coordination through a closed-loop referral system.


In addition to direct client support the Community Nutrition Navigator leads nutrition-related informational sessions and groups collaborates with care teams and assists management in meeting grant-related deliverables and reporting requirements. The position requires accurate documentation of services support for HRSN reimbursement processes and active participation in culturally grounded wellness efforts. This role serves as both an advocate and educator ensuring clients receive respectful comprehensive and culturally specific care that supports their long-term nutritional and overall well-being.

What you will do:

Answer and respond to inquiries in the resource/call center.

Provide patient navigation through health care systems to eliminate barriers to care.

Assist patients in navigating nutrition-specific social service resources and programs including but not limited to SNAP WIC food banks home-delivered meals and culturally appropriate food supports.

Maintain a high level of knowledge on all internal NARA NW programs qualifications and intake processes with particular attention to those with nutrition and food access components.

Assist patients in connecting with appropriate internal programs including nutrition counseling diabetes education and traditional wellness services.

Complete documentation in a closed-loop referral system.

Submit necessary reimbursement/billing information related to HRSN (Health-Related Social Needs) services.

Build productive relationships with internal and external community partners particularly those offering food security and nutrition-related services.

Help patients keep track of upcoming appointments schedule rides for appointments and support with understanding aftercare visit information including nutrition-related recommendations.

Attend trainings conferences workshops and other meetings as directed by program directors.

Coordinate appointments with providers to ensure timely delivery of diagnostic and

treatment services especially for those addressing nutrition-sensitive conditions (e.g. diabetes obesity malnutrition).

Arrange language translation or interpretation services.

Facilitate linkages to follow-up services including dietitian consultations traditional food programs or chronic disease management groups.

Assist clients in learning about and accessing cultural resources and activities including traditional foods and nutrition education rooted in cultural practices.

Monitor well-being of clients in the community setting on a routine basis as determined by the service plan (outreach) with attention to food security and nutrition status.

Send track and follow up on referrals to internal and external resources including emergency food resources nutrition assistance programs and culturally specific food providers.

Provide a warm handoff for clients being referred to internal and external resources (e.g. attending first appointments at food distribution centers diabetes classes or nutrition counseling sessions).

Coordinate care actively initiate and maintain communication and collaboration with all members of the clients support system and service team as permitted by the client including dietitians community health workers and nutrition resource providers.

Maintain thorough accurate and timely documentation as required by agency policy and OARS.

Coordinate and plan nutrition-related activities in alignment with grant requirements ensuring program goals and deliverables are met.

Track and document all nutrition-related services and activities for grant reporting purposes.

Lead or co-facilitate nutrition-focused informational sessions and support groups for clients and community members.

Work closely with management and grant administration teams to support reporting evaluation and continuous improvement of nutrition programs.

Participate in multi-disciplinary team meetings to coordinate care especially for patients with nutrition-sensitive health needs.

Attend all mandatory meetings and clinical supervisions.

Works collaboratively and maintains professional boundaries with clients other NARA staff team members and treats everyone with respect and dignity at all times.

Qualifications

We would like to hear from people that have:

High school diploma or GED required; associate or bachelors degree in public

health nutrition social work or a related field preferred.

At least 2 years of experience in patient navigation care coordination community health or a similar rolepreferably with a focus on nutrition food security or chronic disease prevention.

Certification as a Community Health Worker (CHW) within 6 months of hire.

Knowledge of local state and federal nutrition programs (e.g. SNAP WIC food banks) culturally specific food resources and traditional food systems.

Must pass a pre-employment or for cause drug tests.

Must pass criminal background and DMV checks.

Current CPR certification (within 30 days of hire).

Must have a valid drivers license current insurance and reliable transportation; required to be legal to drive per organizational policy.

5. PREFERRED EDUCATION AND EXPERIENCE:

Experience working within the Native American/Alaska Native community

Skilled in leading educational or informational sessions and facilitating group discussions.

Proficiency in Microsoft Office Suite (Word Excel Access PowerPoint Publisher) database systems (e.g. Athena Practice Connect Oregon) and electronic documentation tools.

Ability to track and document services for reporting compliance and grant-related outcomes.

Available to work some evenings weekends and special events as needed.

Ability to travel between multiple sites or community locations within a day.
