About the Role
Based at our Clarendon Clinic which is centrally located and close to multiple public transport routes this is a great opportunity to increase your skills in community mental health. You will provide a high standard of clinical assessment treatment and outreach support to clients of the St. Vincents integrated adult MHS and their carers demonstrating excellence in discipline specific and recovery-focused case management skills
You will join a dynamic Mobile Support Mental Health Team operating under a recovery-oriented model of care utilising the Strengths Model of Case Management.
- Work within the Strengths Model of recovery orientated practice
- Provide a discipline specific service to consumers who are currently registered with the Clarendon Mobile Support Team
- Assist with and undertake the assessment provision of treatment and delivery of targeted interventions to consumers with complex mental health related needs and their family/carers in line with SVMH policies and strategic direction
- Work a 7 day rotation roster
Classifications for this position will range from
- OT/SW Grade 2 YB20 - YB23 or YC42 - YC45 ($88030 - $per annum)
- RPN Grade 3 NP81 - NP74 ($100932 - $112091 per annum)
Your Contribution
- Bachelor Degree in Occupational Therapy or Social Work
- Division 3 or Division 1 Nurse with Psychiatric Endorsement from the Australian Health Practitioner Regulation Agency (AHPRA)
- A minimum of at least 2 years post qualification within your profession preferably within the mental health field
- Experience in the provision of assessment discipline specific intervention and case management services to consumers with a serious mental illness and complex needs and their carers within a recovery framework
- Highly developed interpersonal and communication skills
- Post-graduate qualifications in relevant area of specialty or in the process of achieving the same are desirable
- A valid Nationally Criminal History Check or willing to obtain

LOCAL WORK ENVIRONMENT
St. Vincents Mental Health (SVMH) includes an adult area mental health service for the inner city areas of Yarra and Boroondara. The adult mental health service components are: The Footbridge Community Care Unit (CCU) the 44 bed Acute Inpatient Service (AIS) located on the St. Vincents Hospital site the North Fitzroy PARC and two community mental health services (CMHS) at Hawthorn and East Melbourne (Clarendon CMHS). Community mental health services comprise the Triage and Crisis assessment and Treatment (CAT) Service and the Primary Intervention Care Team (PICT) includes Extended Triage and HOPE (Suicide Prevention) Continuing Care (CCT) Mobile Support and Treatment (MST) and Homeless Outreach (CHOPS) services. A Consultation & Liaison Service is provided to the health service. NEXUS Dual Diagnosis Service the Body Image and Eating Disorders Assessment and Treatment Service (BETRS) and NEVIL Training Cluster are regional services. Statewide services are the Victorian Dual Disability Service and the Victorian Transcultural Mental Health. The Acute Inpatient Service includes 5 beds designated as the Koori State-wide Inpatient Service and linked to the Victorian Aboriginal Health Service.

POSITION PURPOSE
The Mobile Support and Treatment Service provides intensive home and community-based recovery oriented services seven days a week extended hours for people with a significant mental illness and associated high-level disability needs who require additional clinical support than is available through continuing care services. The multidisciplinary team provides a range of clinical and therapeutic interventions using the recovery-focused Strengths Model of Recovery-Oriented Practice. This position will be required to work a rotating roster over seven days a week and work extended hours.
Under the guidance of the Team Manager and Team Consultant Psychiatrist and in conjunction with team members the Enhanced Care Clinician will provide and coordinate short to medium term additional specialist targeted therapeutic interventions to consumers that are receiving case management services. Interventions provided will be aimed at supporting psychosocial recovery fostering individualised recovery independence and improved health outcomes for consumers that have been identified as having severe and complex needs. Interventions provided could include but not be limited to cognitive behavioural therapy family therapy peer support interventions focused on activity/vocation or additional treatment interventions to meet physical health or drug and/or alcohol needs.
POSITION DUTIES
- Work within the Strengths Model of recovery orientated practice.
- Build a therapeutic alliance with the consumer and family/carers and actively engage them in the planning delivery and evaluation of their care.
- Actively participate in daily handovers/clinical meetings that determine day to day clinical priorities for the team.
- Undertake the role of Duty Clinician and Code Grey Coordinator as required
- In collaboration with the treating team undertake the assessment coordination and provision of treatment and delivery of targeted evidence-based interventions to consumers with complex mental health related needs and their family/carers in line with SVMH policies and strategic direction.
- Utilise discipline-specific skills as appropriate and as required by consumers their family/carers and the Service.
- Assist in the provision of primary secondary and tertiary consultation services community development and education liaison and linkage to other agencies that also provide support to people with mental illnesses as required. This includes participating in and contributing to locally held portfolios.
- Display active participation in professional development and ongoing learning opportunities.
- Provide case management to cover secondary case-loads for other case managers when required.
- Attend and participate in discipline specific meetings and display a commitment to attending discipline specific training.
- Participate in Line Management regularly with the team manager
- Display a high standard of clinical practice and professional conduct at all times.
- Work collaboratively within the multi-disciplinary team.
- Ensure that all reporting information is coordinated and completed as required.
- Support the work of other partners in care MHCSS and primary care providers and ensure their participation in the consumers clinical care.
- Provide professional supervision to staff and to students on placement as regulated within the discipline (this does not apply to Psychologists with classification P2).
- Maintain all documentation in line with SVMH policies including clinical documentation outcome measures and CMI reporting.
SELECTION CRITERIA
ESSENTIAL REGISTRATION LICENSE OR QUALIFICATION REQUIREMENTS
- A minimum of at 2 years post qualification clinical experience within Mental Health
- Registration with Australian Health Practitioner Regulation Agency (all disciplines except Social Work).
- Social Workers - An approved degree in Social Work and eligibility for membership of the Australian Association of Social Workers
- Current Victorian drivers license
OTHER ESSENTIAL REQUIREMENTS
- Clinical experience within Mental Health Services.
- Experience in the provision of assessment discipline specific intervention and case management services for people with serious mental illness and complex needs and their carers.
- Demonstrated ability in working as part of the multi-disciplinary clinical team in promoting a standard of excellence in clinical practice. This includes contributing to team clinical processes such as Clinical Review and supervision for staff and students.
- A comprehensive understanding and working knowledge of the Mental Health and Wellbeing Act 2022 (including amendments) and other relevant legislation.
- Commitment to work collaboratively and effectively as part of the multidisciplinary team with multiple key stakeholders both internal and external of the St Vincents Health organization (including consumers carers family members and community agencies).
- Demonstrated ability to working collaboratively with clients and carers to promote self-determination and enable progress towards reaching personal goals.
- Highly developed interpersonal and communication skills including problem solving ability and conflict resolution skills as well as sound oral and writing skills.
Occupational Therapy:
- Knowledge of and commitment to providing a comprehensive range of occupational therapy assessments and interventions for people with serious mental illness and complex needs and their carers.
- Understanding of evidenced based practice and models of occupational therapy (including the Model of Human Occupation) and their application in clinical practice.
- Knowledge of sensory modulation theory and intervention.
- Knowledge of the specific assessments including the MOHO Assessments and Assessment of Motor and Process Skills (AMPS).
- Knowledge of appropriate medication and administration management including relevant legislation
- Ability to apply nursing models and processes
- Awareness of infection control techniques
Social Work:
- Knowledge of and commitment to providing a comprehensive range of social work assessment and intervention services for people with serious mental illness and complex needs
- Demonstrate knowledge of family and social system assessments with the ability to identify gaps and facilitate access to appropriate resources
- Knowledge of and commitment to providing a comprehensive range of psychological assessments including diagnostic cognitive and personality assessments for people with serious mental illness and complex needs.
- Experience in the development and application of evidence based therapeutic interventions for people with major mental health disorders.
- Comprehensive understanding of the theories of human behaviour and psychopathology and their application in clinical practice
Nursing:
- Knowledge of appropriate medication and administration management including relevant legislation
- Ability to apply nursing models and processes
- Awareness of infection control techniques
Psychology:
