Bilingual Case Coordinator for Individuals With Disabilities
Full Time/Nonexempt Position
Pay: $50000 per year

JOB DESCRIPTION: Care Coordinators coordinate in-home and community-based services for the elderly and people living with all types of disabilities.

Duties to include:

1. Working Mon-Fri 8a-5p. Please note the position will occasionally require the manager to work a slightly different schedule to accommodate client needs. This may include working direct care shifts some evenings/ weekends.
2. Performing on-call duties at least once every 10 weeks. The duties last for a duration of two consecutive weeks. These duties include answering and managing emergency phone calls from 5p-8a M-F and all day Sat and Sun. These duties may also include working direct care shifts.
3. Maintaining and growing a caseload
4. Scheduling shifts for clients and direct care staff
5. Orienting and training direct care staff with clients
6. Monitoring direct care staff performance
7. Holding staff meetings as needed and attending weekly program department meetings
8. Conducting intake of new clients
9. Meeting with clients/families minimally every quarter
10. Attending meetings with other agencies involved in a clients care
11. Ensuring that all paperwork in client files is updated regularly
12. Documenting all significant conversations and/or correspondences with staff and clients
13. Maintaining client and agency confidentiality at all times

QUALIFICATIONS:

1. 18 years of age or older
2. Ability to obtain a current CPR/FA certification
4. Ability to obtain fingerprint clearance
5. Ability to clear a DES background check
6. Ability to obtain a current T.B. clearance card
7. The ability to work in a team environment
8. The ability to handle difficult situations with clients/families
9. The ability to work a flexible schedule
10. The ability to learn and operate UCPSA program software
11. The ability to work independently
12. The ability to lift and transfer clients
13. Own/have regular access to and be able to operate a motor vehicle for the purpose of visiting and transporting clients attending meetings outside of the office etc. Proof of current registration and insurance of the vehicle must be kept on file with UCPSA
14. At least one year of direct care experience personally or professionally a degree in a related field and/or case management experience
15. Bilingual in English and Spanish is required
